BREAST PAIN APPEARED WHEN BABY'S TEETH STARTED TO ERUPT?

This is my 10 month-old patient I saw online 2 months ago. Mom was complaining of sharp shooting pain on the left breast right after feeding for 1 week at that time. It started when baby's teeth started to erupt. There was a ni**le bleb. During online consultation, it was hard to assess this baby and advised them to see me face-to-face. We managed the ni**le bleb.

One week after their online consultation, mother was still complaining of ni**le pain after breastfeeding but shorter. When I assessed baby's mouth, there was a type 4 tongue-tie that could have contributed to recent ni**le pain. We performed frenotomy that day and advised them to do oral exercises.

A week after frenotomy, mother noticed that the pain was gone only to reccur 3 days after with reappearance of bleb. I advised them to improve the stretching and exercise and managed the new bleb.

One month after frenotomy, they followed up enjoying breastfeeding pain-free even prior to total disappearance of milk bleb. Patient had good appetite with solid food and crawling on fours. Praise God 🙏

Why breast conditions like ni**le bleb appeared only on a baby with tongue-tie with erupting teeth ?

Tongue-tie restricts tongue movement, affecting latch and sucking patterns. Erupting teeth change oral mechanics, potentially increasing pressure or altering forces on the ni**le. With teeth present, if the tongue's movement is restricted, the baby might apply different pressures or bite patterns causing ni**le trauma or duct blockage.

Treating the breast condition alone without addressing the main cause which is baby's shallow latch (due to a tongue-tie) is like treating it like a bandage solution. Yes it can mprove but it will just reccur.

Ang isang breastfeeding counselor like Ms. Lorena Rivera at ng buong team nya sa Zalor Mom-Infant Care Services ay maaaring magbigay ng mga sumusunod na serbisyo:

Mga Serbisyo
1. *Pagsusuri sa pagpapasuso*: Tinitingnan ng Taga-gabay, kung ang sanggol ay nakakakapit ng maayos sa suso ng ina at kung ang pagpapasuso ay walang problema.
2. *Pagtuturo sa tamang pagkapit*: Itinuturo ng breastfeeding counselor kung paano ang tamang pagkapit ng sanggol sa suso ng ina.
3. *Pagtulong sa mga problema sa pagpapasuso*: Tinutulungan ng breastfeeding counselor ang mga ina na may mga problema sa pagpapasuso, tulad ng masakit na suso o hindi sapat na gatas.

HOW BREASTFEEDING PROTECTS YOUR BABY?

I remember during the pandemic, there's a breastfeeding momwho tested positive with COVID together with 2 older kids. They all experienced fever, weakness, diarrhea and loss of taste and smell.
She thought it was just a regular flu and she's been playing and kissing her baby for 5 days until she experienced loss of smell and taste and confirmed positive on the same day. What's amazing according to her was her 6-month-old breastfeeding baby remained asymptomatic , feeding well and playful when they all tested positive.

When you think the youngest and the most vulnerable baby will be the sickest in the family during flu season, mother's breastmilk serves as shield via entero-mammary pathway (when pathogens are ingested) or broncho-mammary pathway (when pathogens are inhaled). Antibodies are made by mother's body after exposure to these pathogens and it was made available to her baby thru breastmilk.
