Kapag pupunta sa ospital para sa check-up, admission, o anumang serbisyong medikal, huwag kalimutang dalhin ang mga mahalagang dokumento, lalo na ang:
✅ 1 Updated Valid ID - Halimbawa: Driver’s license, Passport, PhilHealth ID, atbp.
✅ PhilHealth ID / Number - Kung mag-aavail ng PhilHealth Konsulta o PhilHealth Services
✅ Birth Certificate - para sa mga pasyenteng 17 y/o at pababa
✅ Marriage Contract - para sa ER at admitted kung kasal (*Kung beneficiary mo sa PhilHealth ang iyong asawa)
📍Siguraduhin na ang mga dokumento gaya ng valid ID ay updated at hindi expired
📍Ang pagdadala ng mga dokumentong ito ay makatutulong upang mas mapabilis at maging maayos ang inyong transaksyon sa ospital.
📍Maaaring magpunta sa Social Services Department sa Hospital Lobby para sa mga karagdagang impormasyon at mga katanungan.

When visiting the hospital for check-up, admission, or any medical service, please remember to bring the important documents, especially the following:
✅ 1 Updated Valid ID – Examples: Driver’s License, Passport, PhilHealth ID, etc.
✅ PhilHealth ID / Number – For those availing of PhilHealth Konsulta or any PhilHealth services
✅ Birth Certificate – For patients 17 years old and below
✅ Marriage Contract – For ER and admitted patients who are married (*If your spouse is listed as your PhilHealth beneficiary)
📍 Please ensure that your valid ID and other documents are updated and not expired.
📍 Bringing these documents will help speed up and streamline your hospital transactions.
