Pleast print out your insurance form for faster transactions para sa mga gustong gumamit ng kanilang HMO. Lesser hassle na po kasi for you pag nasa hospital na pag naka ready na po yung form.

Inuulit ko po, it is better to have your forms printed before you visit the hospital.

📍Kailangan ko po ng medical certificate. Magkano po?
Before the issuance of medical certificates, all patients should undergo a check up. Medical Check up or Consultation is P500. There is a minimal additional fee for medical certificates or Fit to Work (depends on the patient’s case)
