📢 ANNOUNCEMENT: MEDICAL CERTIFICATE FOR OJT STUDENTS

All OJT students are required to secure a Medical Certificate from the University Health Services (UHS) before deployment.

✅ An endorsement letter from CARDSIS must be submitted to the UHS.
✅ Answer the medical assessment Google Form from your OJT coordinator
✅ Bring your student ID/registration form and recent chest X-ray (within 6 months)
✅ Undergo basic health screening

⚠️ Students with medical findings, recent hospitalization, or pregnancy must present another certificate from their attending physician for clearance.

Certificates without UHS seal are not valid.
No online or messenger requests will be accepted.

In light of the recent rise in respiratory illnesses, Bulacan State University will be implementing health and safety measures starting Monday, October 20, 2025.

🩺 Key Guidelines:
1️⃣ Mandatory Face Masks in all enclosed spaces (classrooms, offices, etc.)
2️⃣ Daily Disinfection of classrooms, offices, and laboratories
3️⃣ Self-Reporting of flu-like symptoms (cough, fever, sore throat) to the University Health Services
