Patient Information:
• Age: 27 years old
• G1P1
• Intrauterine Device (IUD) in place for 7 years

Chief Complaint:
• Heavy vaginal bleeding and pelvic pain

Assessment/Diagnosis:
• Abnormal Uterine Bleeding (AUB) secondary to prolonged IUD use
• Dysmenorrhea / Pelvic pain associated with IUD

Plan:
1. Remove IUD under aseptic technique.
2. Counsel patient on alternative family planning methods.
3. Shift to injectable contraceptive DMPA 150 mg IM every 3 months after ruling out pregnancy and infection.
4. Provide health teaching on possible side effects of injectable contraceptives irregular bleeding, weight gain, delayed return of fertility.
5. Advise follow-up for monitoring of bleeding pattern and general well-being.
