CANCELLATION POLICY

Due to limited slots of patient per day, we ask that you provide us 24 hours notice for the cancellation of your schedule.

As courtesy, patient who do not honour their appointments will be charge.

New Dental Protocol’s

STRICTLY BY APPOINTMENT/ No Walk in
You can call or message us and have your appointment scheduled.

TRIAGE
We will screen the patient through phone call, messenger or other non contact means 1to2 days prior to treatment.

Please fill up the Patient’s Screening form and Informed Consent form upon arrival to the clinic.

Toothwellness Dental Center is now located @ 006 Trinidad St., San Isidro Subdivision, Pamplona Uno. Just beside the Barangay Hall near Christ the King Hospital

In compliance with the New Normal, we have some REMINDERS TO ALL OUR PATIENTS:

1.No Face Mask No entry Policy

2. We Provide Shoe Sanitizing Mat and Spray.
Please sanitize your shoes before entering the clinic.
Use Disinfectant Spray before wearing shoe boots. Sanitize your hands using 70% alcohol.

3. Mandatory Temperature Check and Oximeter reading before entering the clinic.

4. Only the Patient is allowed inside the dental clinic.
(For Children, Senior Citizen & Patients with special needs, only one companion is allowed)

5. Always maintain Social Distancing and avoid touching your face.

6. There will be a 30minutes to 1hour interval between patients, to perform infection control protocol.

7. Be on time!
To allow us to review your forms, wearing of PPE’s and for disinfection before we start the treatment.
We apologize that if you came late to your appointment, you have to re-schedule.

8. Preoperative antimicrobial mouthrinsing must be done. (30secs to 60secs)

9. After Treatment
Remove your isolation gown, shoe boot and head cap place it in the Bin.

10. Protective Personal Equipment
As a preventive measures while performing dental procedures will be utilized.
DENTIST, ASSISTANT and PATIENT will wear a PPE that will help to protect from any risk of infection.

11. The Philippine Dental Association (PDA) has set an standard fee in all dental procedures with the cost of infection control (PPE)
