The rollout for the COVID-19 BIVALENT vaccination for Healthcare Workers (A1-A1.9) Elderly Persons A2(Senior) and A3- with Comorbids will start on 31 July 2023, Monday, 1-4 PM (50 SLOTS DAILY) at the JBLMGH OPD Bldg.

Please take note of the following:
1. All vaccinees with an interval of at least 4 months from Primary Dose (1st and 2nd dose) are eligible for Bivalent Vaccine.
2. Pre-registration is strongly encouraged using this link https://bit.ly/3Xu7duv
3. Print FORMS: Informed Consent & Health Declaration Form.
4. Bring Valid ID, ALL previous Vaccination Cards, and Vaccination forms.
5. Arrive at the vaccination site at least 30 minutes before your scheduled appointment which will be seen on the printed form.
6. Observe minimum health protocols while inside the hosp premises.

There is a limited supply of vaccines allocated to JBLMGH. We will be enforcing 1st come, 1st served basis.

We have more than 20 TB space in our network attached storage (NAS) in RAID configuration, cloud backup and backup using multiple external SSD drives, all with Bit locker drive encryption protection. Our network is being maintained regularly by our exceptional IT consultant, Mr. Jordan Lucas.
