Chronic bronchitis and emphysema are two major forms of COPD with distinct clinical patterns. Chronic bronchitis is defined by a long-standing productive cough caused by airway inflammation and excess mucus production. Patients often present as “blue bloaters,” with cyanosis, obesity, crackles or wheezes, and complications such as hypoxemia, pulmonary hypertension, and cor pulmonale. Gas exchange is impaired primarily due to ventilation–perfusion mismatch.

Emphysema, on the other hand, is characterized by destruction of alveolar walls and loss of elastic recoil. These patients are often “pink puffers,” appearing thin with dyspnea, pursed-lip breathing, hyperinflation, and minimal cough. Oxygenation is relatively preserved until later stages, but complications include pneumothorax and significant weight loss from increased work of breathing.
