MASTECTOMY WHO!?!

My patient was told she would never be able to lift more than 3 lbs. again! Let me tell you nothing is impossible with the proper exercise progression and of course a persons will and determination to get back to normal!

Due for surgery with the goal of being able to walk, FIVE fall histories that led to her fear of falling, not gaining progress in another rehab facility, and ready to try just about anything else when I advised another bout of conservative management. Two sessions later, this is Mrs. C's first few steps months after being wheelchair bound. No surgery. No fancy machines. It's amazing what your body can do if you just let go of the fear and start. πŸ’―

- I was told to rest my back and to stop doing sports.
- I've learned to live with it.
- It'll go away on its own.
- It's too painful to move so I just don't.
- I was just given pain medication.
- I'm too old to exercise anyway.
- I was given an orthotic to improve my posture.
- I don't have the energy.
- It's because I work long hours..
- It's because of my posture, I use my cellphone a lot.

As Physios, it is essential to keep up with current evidences to address different cases we might face in the clinics or during house visits. It is for the benefit of the patient to be properly educated about their case or condition. There is fear of the unknown. AND PATIENTS ARE ONLY USUALLY TOLD WHAT NOT TO DO. They don't know what they CAN do and end up not doing anything at all.

Therefore, here are a few questions you might want to ask:

What can I expect during the session?
What do the Physios expect from their patients?
Can the pain be addressed?
Is there hope?
How do we start when I can't even move?
Am I allowed to exercise????????

Have you ever been faced with a person and immediately think "he/she will be prescribing Therapeutic Ultrasound, ES or TENS."

I bet you have! Let's be honest, who hasn't?

Their Rationale: "You will need ultrasound to decrease the pain on the (part) so they can move the (part) freely".

False.

What do you think of this rationale?

Past journals often conclude passive modalities MAY BE used as an adjunct to your exercise prescription but the current EBP believe that these passive modalities are more "quickfix" and "placebo".

Personally, we can do without. I did.
