💉For A1.8 OFW's Departing within 4months. Must bring documents as proof that you are to depart within 4months.

💉For A1.9 Dependents of Health Care Workers, must be the same household or living together with the Health worker. Bring endorsement letter with photocopy of HCW's ID

💉For A5 and A5 dependents/beneficiary: Please bring 4P's ID

💉Don't forget to bring the following:
1. Ballpen
2. Face shield
3. DQR / Valid ID
