Itinawag ako ng ER dahil may isang 70 year old F, na sumasakit ang kanang bahagi ng tiyan tagos hanggang likod. Matagal na po siyang may . Matagal na rin nananahimik at ngayon lang sumakit.
Nung pinuntahan ko siya, mataas ang BP, masakit ang ulo, kumakalabog ang dibdib at masakit ang tiyan.
Mataas ang bilirubin at lipase.
Ito ay isang case ng acute choledocholithiasis with gallstone pancreatitis.
Kailangan niya ng at .
Ngunit, may isang problema. At ito ay parating nakakalimutan na itanong ng mga doctor. - Ang pasyente ay nagtatake ng ASPIRIN.
Pagnaka Aspirin, hindi po pwedeng operahan dahil duduguin Tayo. Kailangang itigil ng 7 araw bago pwede operahan.
Kaya mga pasyente, Sabihin lahat ng maintenance medication sa doctor kung mag papa opera.

Another day, another problematic case.
This patient had her done for or last October 2025 and waited until January 2026 to have her removed.
I was keeping my fingers crossed the whole time.
ERCP cholangiogram says there is no opacification of the cystic duct. So I expected this to be a difficult one.
Today, as expected, the cystic duct is huge! Dilated to about 1cm and hard! It was filled with stones!
After patiently dissecting and finally decided to use a tie instead of clips, the surgery was done and successful. Took around 3 hours to finish!
Again, thank you to my team AMC for always being there and helping me.

Tough day today. I thought my cases today were straightforward and relax but it turned out that it was tough and difficult. Good thing I have the best team on my side to help me get through the toughest challenges!

I am truly honored and grateful to my patient who came all the way from the UK just to have me do her laparoscopic cholecystectomy.
Although what seemed to be a simple lap chole, turned out to be an acute choledocholithiasis. patient had to undergo an then eventually the planned .
She is now up and about but I need her to stay in the Philippines for a week just to make sure everything is ok before I allow her to go back to the UK.

Ngayon, ginagawa ko naman isa isa yung mga na ERCP.
So far so good! Successful naman lahat.

Follow up on yesterday’s case.
After proper timing and treatment, we successfully brought down the patient’s lipase from 41000 to 283! Whew!
Then was done and the culprit was taken out of the bile duct.
Nakangiti na ang pasyente at kumakain na rin.
Ang Plano ay ….
After 72 hours, papa na siya para iwasan may mahulog na bato mula sa apdo.
Abangan ang part 3….

First problematic case of 2026.
Sa mga nagsasabing maliit lang ang bato sa apdo, at ayaw magpaopera. Ok lang naman yan dahil yung may katawan ang may desisyon sa sariling kalusugan.
Pero dapat niyo intindihin na ang maliit na bagay ay Kayang lumusot sa maliliit na lugar.
Katulad itong pasyente na tumawag sa akin ang nanay dahil nag delikado na ang kanyang a**l.
Pinakita sa akin ang laboratory, na ang Taas Taas na ang SGPT at Pati ako natakot sa value ng kanyang lipase. 41000+!
Ang ibig sabihin niyo ay ! Hindi lang sa bile duct nahulog ang bato, kundi sa pancreatic duct napunta at naging full blown pancreatitis!
Pagdasalan natin na maligtas ang pasyenteng ito.

Back 2 back cases of surgery with liver problems.
First case is gallstone in the setting of liver cirrhosis and
Second case is gallstone with perihepatitis. Note the violin string sign adhering from the liver to the peritoneum.
Kaya, alagaan natin ng mabuti ang atay natin.
