Tignan niyo ito, pati ako na-istress na ng sobra.
1. Nung October 20, 2025 sumakit at sinabihang Kelangan na operahan. Ayaw maniwala. Umuwi.
2. Nung November 20, 2025 bumalik sa ER dahil napaka sakit. Magpapaopera na daw.
3. Nung pinuntahan ko sa kwarto, Hindi ko gusto kulay niya. Pinilit niya ganyan talaga kulay niya. So para walang sisihan, pina blood test ko. Ang result, sobrang taas ng SGPT, ALKPO4 at Bilirubin. So nirefer ko para ma ERCP.
4. So na ERCP siya at nakita na may bato nakabara. Maniwala ma sa akin na hindi normal ang kulay niya.
5. Ngayon, ubos na pondo ng HMO. Kaya umuwi nalang sila…..

Napakabilis ng fast break!
Natutuwa naman ako sa pasyente ko. Galing pa siya ng Bohol para magpaopera sa akin. Nagawan ko naman ng paraan para maischedule siya kaagad. Hindi pa nakakaraan ng 24 hrs after na maoperahan, nag tetext na sa akin ng ganito.

Imagine this, a 115kg patient with a 2.5cm gallstone impacted at the gallbladder neck with thickened gallbladder wall. And the surgeon is just 65kg.
Tapos ang gagawin is open chole?
Ang laki kaya ng magiging hiwa niyan and how many assistants are needed to perform a successful surgery.
Buti nalang hindi ako nag oopen kundi laparoscopic cholecystectomy na 3-port. I only need 1 assist to show me the way! Salamat Mother Josie! And the wounds are only 3 small incisions that are 5mm long.
Patient is awake immediately after surgery and can eat after a few hours.

Ding! Ang Bato! ni Darna!!!!
This is a 2.5cm perfectly round golden mixed gallbladder stone that is impacted at the gallbladder neck causing inflammation at the triangle of calot making dissection a bit bloody.
Case was done in 45 minutes.

Making sure that there are only 2 structures left after carefully dissecting the gallbladderall around… The cystic duct and cystic artery are the 2 structures left holding the gallbladder to the common duct. We do this to avoid injury to the bile ducts.
It is also very important that the assist knows what you are doing and he serves as fail safe assist to avoid untoward injuries.

The importance of a very meticulous dissection only possible with a high definition camera. If you see the picture where the red X mark is, it is easy to mistake this for the cystic artery and clip it. If this artery is clipped, 25% of the blood supply to the liver will be gone. Good thing I saw that the artery curved into the liver and this proved to be the right hepatic artery. The cystic artery is the one on top doubly clipped.
Thank you to my guardian angel who showed me the way and avoided a drastic mistake.
