Malignant Spinal Cord Compression

Key Points:
1. For a cancer patient presenting with back pain, cord compression should always be part of the differentials.
2. For back pain with a neuro exam suspicious for myelopathy, there should be a low threshold to start steroids and do MRI of the spine ASAP.
3. For documented malignant cord compression, we use the Spinal Instability Neoplastic Scale/Scoring (SINS) to determine if we need to operate on the patient or other less invasive treatments like radiation therapy could be given.

Aside from administering intrathecal chemotherapy which has in my opinion an unpredictable response, there is a medicine which is now available in the Philippines for HER2+ breast cancer which has a good brain activity.
