Nodules on the face ✨️ My patient noticed slow-growing nodules on the face that were itchy. For these kinds of lesions, we would need to do a skin biopsy to arrive at a definite diagnosis and possibly blood tests to check for other co-morbidities. We would also need to refer her to other specialists for co-management if necessary.

Acne. ✨️ Acne vulgaris (commonly known as acne, breakout, or pimples) is a common skin condition that occurs when follicles become plugged with oil, dead skin and bacterial invasion, that can cause inflammation which could possibly lead to hyperpigmentation and scarring. It can be triggered by trauma, certain medications, hormones or infection. It can be located anywhere like the scalp, hands and face. Dermatomyositis. ✨️ This is an inflammatory disorder of the skeletal muscles and associated skin lesions. If there is a patient with (proximal) muscle weakness and presents with a rash on the face (heliotrope rash), shoulders and chest (shawl sign), photosensitivity and lesions on the hands especially on the joints of the hands (Gottron's papules), and dilated blood vessels around the cuticles (nail fold changes), we would need to do certain tests to confirm the diagnosis and then provide treatment options for the patients. Cutaneous larva migrans. ✨️ It is a parasitic skin infection caused by hookworm larvae from cats, dogs and other animals but can also infect humans through skin contact from walking barefoot on the beach or exposure from moist soft soil that has been contaminated by animal f***s. Most of our patients have either been farmers or have been on vacation on the beach and have picked up the larvae from direct skin contact.

Typically the lesions start out as an itchy rash with bumps that later progress and having a snake-like pattern, showing the tracks where the larvae have been migrating (hence, "migrans").

I encountered this patient who had not been to the beach but was exposed through either gardening with soil frequented by stray cats or from contact with the stray animals themselves.

It is important to be vigilant when exposed to contaminated areas like the sandy beaches or moist soft soil and set an appointment with a board-certified dermatologist if you suspect you may have this kind of lesion.
