Putatan Emergency Response Service EMS Volunteer RULES REGARDING THE USE OF BARANGAY AMBULANCE

1. It shall transport patient with emergency cases to the nearest hospital.

As a general rule, the ambulance service provided by Sanguniang Barangay ng Putatan is for Emergency Use Only. Use of the ambulance is within Metro Manila only.
2. Residents of Barangay Putatan shall have first priority with the use of ambulance and the Putatan Emergency Response Service Personnel shall attend on the severity of the patient’s case
3. Patients requesting services must present proof of residency in Barangay Putatan by presenting any of the following a VALID ID, BARANGAY CLEARANCE or COMMUNITY TAX CERTIFICATE.
4. Patient must sign the Acknowledge Form accepting the policies regarding the use of Ambulance
5. Patient shall shoulder the cost of the SLEX toll fee (one way only) whenever required
6. Patient who is not resident of Barangay Putatan must shoulder fuel expenses for a maximum of Ten (10) liters, if the use of Ambulance will go beyond the city of Muntinlupa area
7. for transfer of patient from one hospital to another, the attending physician must coordinate the transfers with the physician of the receiving hospital
8. For transfer of ICU patient, the patient must be accompanied and endorsed by a DOCTOR or NURSE to the receiving hospital
9. For transfer of patient from hospital to their residence, the patient must secure a DISCHARGE SLIP from the hospital after settling of their account/hospital bill
10. A maximum o TWO (2) companion is allowed to accompany the patient inside the Ambulance.

Isang mapagpalang araw. Ilang taon nanaman ang lumipas. taon na kung saan ating nabuo ang group na ito naging comunication sa ating mga pangarap naging daan upang malaman ang mga bagay bagay dahil sa group page na ito. Muli akung babati ng magandang araw sa inyo ng lahat.

Dahil po sa kahilingan ng lahat ang bawat disisyon ay kailangan pang kalahatan at inaaprubahan ng bawat isa. Naka pag deklara po ako kahapon ng ipag paliban ang ating Xmas party na dapat dec 21 ginawa ko pong dec 23 kadahilanan po sa isang pang yayari na di po inaasahan.

Sa pag kakataon ito muli pong ibibabalik ang date po ng Dec 21 6pm Brgy covered court putatan. upang ganapin ang X Mas party ng PERS. KABALIKAT CIVICOM. EMT Volunteer . at muli akung humihingi ng paumanhin at pang unawa sa pang yayari.

Maraming salamat po at maligayang pasko at masaganang bagong taon sa inyo.
