900 ang maaring mabakunahan kada araw ng 1st dose

Para po sa mga hindi pa nababakunahan, maaari po kayong magtungo ngayong araw hanggang bukas sa Rizal Triangle, 9am to 4pm.

A1 - Frontline Health Services Workers - PRC ID or any valid ID with picture, birthdate and Certificate of Employment
A2 - Senior Citizens - OSCA ID or any valid ID with picture and birthdate
A3 - Persons with Comorbidities - Any valid ID with picture and birthdate at medical certificate/ reseta or basyo ng gamot
A4 - Non Medical Frontline Health Workers - Company ID or any valid ID with picture, birthdate and Certificate of Employment.
A5 - Indgent Population - Any valid ID with picture and birthdate at Certificate of Indigency na makukuha sa inyong barangay.

As of now, our priority for the testing are symptomatic patients referred by hospitals and our barangay health centers/disease reporting units. But we may schedule individuals needing RT-PCR tests as a requirement for elective surgeries and other medical procedures. Appointments for the RT-PCR test are made through this email address. Our laboratory follows the DOH Guidelines on Expanded Testing for COVID-19. Below are our operational schedules for your reference. Thank you.

1. Swabbing Facility at Gordon College: Monday to Thursday, 9:00 AM to 3:00 PM, Friday 9:00 AM to 11:00 AM

2. Olongapo City Molecular Diagnostic Laboratory (NO SWABBING OR SAMPLE COLLECTION DONE IN THIS FACILITY, TESTING ONLY!): Monday to Friday, 8:00 AM to 5:00 PM

3. All payments for the test shall be shouldered through Philhealth (if patient is a member or a declared dependent of a member) following the guidelines set by Philhealth, provided such claims complies with the provisions of the Department of Health exclusive of the Miscellaneous and Administration Fee (MAF) amounting to Five Hundred (Php 500.00) pesos. If the test cannot be covered by Philhealth, then the actual cost amounting to Two Thousand Seven Hundred (Php 2,700.00) pesos shall be paid. Payment shall be made after requirements have been completed and an instruction will be provided as to the mode of payment.

5. The swabbing team will contact the patient through the contact number provided and will give a confirmation of schedule either through text or call. NO CONFIRMATION, NO SWABBING OR SAMPLE COLLECTION.

6. For companies requiring RT-PCR tests for a group of their employees (as per company policy, prior to work deployment, routine testing, etc.), they may send a letter of request with attached list of employees to be tested and the reason for such request with the details indicated in Number 4 above in excel format.
