1. PSA Negative Result
2. Barangay Certification
3. Voter's Certification or ID
4. National ID
5. Baptismal Certificate
6. Marriage Contract
7. Certificate of Live Birth of Parents
8. Siblings Live Birth
9. Sedula
10. 2x2 picture white background
11. Joint Affidavit of two disinterested person

👵REQUIREMENTS FOR LATE DEATH CERT

✨ 1st Quarterly Municipal Advisory Council (MAC) Meeting ✨

Successfully conducted the 1st Quarterly MAC Meeting, bringing together our dedicated Municipal Officials, partner agencies, and stakeholders to discuss updates, accomplishments, and plans for the continuous implementation of programs—especially for the Pantawid Pamilyang Pilipino Program (4Ps).

This gathering highlights the importance of collaboration, coordination, and shared commitment in uplifting the lives of our community. 💙

Sa pagdiriwang ng Social Work Month, kinikilala ng Pambansang Pamunuuan ng Pantawid Pamilyang Pilipino Program (4Ps) ang ating mga social worker—ang mga “Angels in Red Vest” na walang sawang naglilingkod at nagbibigay malasakit sa mga pamilyang Pilipino.

Sila ang katuwang ng programa sa paghubog ng mas matatag na kinabukasan para sa mga benepisyaryo ng 4Ps—mula sa pangunguna sa Family Development Sessions, pakikipag-ugnayan sa komunidad, hanggang sa paggabay sa mga pamilya tungo sa mas maayos at mas maunlad na pamumuhay.

Ang temang “Social Workers: Uplift. Defend. Transform.” ay sumasalamin sa mahalagang papel ng mga social worker sa pag-angat ng buhay ng mga pamilyang kanilang pinaglilingkuran, pagtatanggol sa kanilang karapatan, at pagsusulong ng positibong pagbabago sa bawat tahanan at komunidad.

Ang ating mga social workers ay patunay na ang serbisyo publiko ay may puso, may layunin, at may malasakit para sa bawat pamilyang Pilipino.
