REQUIREMENTS:

• Curriculum Vitae
• Letter of intent to be submitted to the department chair and training officer
• Transcript of records and Diploma
• PSA Authenticated Birth Certificate
• PSA Certificate if marriage if applicable)
• Comelec certification - Original/Voter's ID
• Medical Certificate from Pasig City Health
Office with original medical results
(instructions to be given once accepted in the program)
