What Is Optimum Care Program?
The CareHealth Plus Optimum Care Program is a 10-year program with a paying period of only 5 years that provides comprehensive hospitalization services, outpatient care and life insurance benefits to its Members.
Aside from the 10-year coverage, Optimum Care medical benefits are extended on the 11th year until the 15th year with Maximum Benefit Limit (MBL) that continues to increase yearly by 5% or 8%. Medical benefits may be assigned to immediate family members up to the Second (2nd) degree of consanguinity/affinity on its extended years.
Who Can Avail The Optimum Care Program?
To avail this program, prospective Members must be from age 7 years old to 65 years old.
What Are The Options For Optimum Care Program?
Members can choose among the two (2) plans that this program offers:
Five (5) Units with a minimum annual premium of P46, 780
Ten (10) Units with a minimum annual premium of P88, 263
