CAREHEALTH TIMELINE FOR COVERAGE
✔HMO Member will be covered once Official Receipt is issued (just present OR Number and valid ID for consultation, ER and confinement) while waiting for the HMO card
✔HMO Card will be available 60 days after submission of complete application form and 2x2 ID photo
✔Pre-existing illnesses will be covered on 13th month onwards
✔Free Annual Physical Exam and Annual Propylaxis (teeth cleaning) can be availed after 3 months of membership
✔Consultations can be availed starting on day one of coverage both for pre-existing and non-pre-existing conditions
