Patient: Doc! I have difficulty breathing to my left nostril. Can you check? I plant to undergo rhinoplasty also kaya I chose you being an ENT Doctor at the same time.
Me: Thank you for choosing me. Let me check sa clinic.
Physical exam: you have a deviated septum starting at the front of your septum up to the back portion (bony septum).
Do you have any history of trauma to your nose?

Patient: Yes po Doc.
I fell during childhood kaya may scar din ako sa right side ng bridge.
Me: Ah.. that explains it. Cge we can easily repair it but making it look better will be challenging.

Patient: Doc, make my nose look more feminine. I dont want goretex on my bridge. Sexy alar please!
Me: Okay I believe you don’t need it (goretex) Mag osteotomy tayo. Pero for the tip.. let’s standby a Mesh as plan B to add structural support (best is rib if feasible financially).

Patient: (immediately postop) Doc! I can breathe very clearly in my left nostril already. Thank you!

Patient: Doc, I don’t want any foreign body for my . I want subtle improvements on my nose leaning more on Korean-based inspo.
Me: Oh okay.. we need to make your nose slimmer.. it will be sloping just so your tip will look more defined.

Patient: so what materials will you use for the no implant rhino?
Me: We’ll use your ear and nasal septum cartilages para no chance of rejection and very very low chance of infection.
We’ll do osteotomy also and defat your tip, sail excision and alar base surgery.

Patient: Doc! It has been more than 2 years! Thank you very much. I can still see my face on my new nose. 😇 I do not see any scars also on my alars!

Patient: Doc! pwde po kaya sa akin yung ganyang peg?
Me: sure ka ha? maiiba ang face mo.. 😇

Patient: Yes Doc I am ready for that change.😊
Me: Okay cge we will need to use a “goretex” for your bridge and probably we’ll need a mesh too.
Gamitin narin natin ang nasal septum and ear cartilages mo.

wait ah.. make sure alam mo PIN code ng fon mo ha just in case hindi ka na makilala after ng natin.😄👃🏼✌🏼

Patient: Doc, can you help me with my nose? ito po talaga ang insecurity ko.

Me: Okay naman surgery ng cleft lip mo maganda.
Yeah, last step in the reconstruction is the correction of the cleft lip nose deformity.
Cge gawin natin but it’s best to use your own rib.
Do you have any problem sa with your nasal airway?

Patient: Yes po Doc, yung left nostril ko sometimes barado.
Me: Okay cge cge let’s do it. It should not only be Aesthetically pleasing but it should also be Functional.

Patient: Doc, I had goretex rhinoplasty with ear cartilage.
Kaso po I am not happy with the result.
It’s been 7 months pero my bridge is still leaning to the left.
I look more matured kasi mataas for me ang new bridge ko.
Me: I see. Have you consulted this with your surgeon?

Patient: Yes Doc, maga pa daw. Pero habang tumatagal mas nagiging obvious na hindi pantay.
Also Doc, I have seen in some of your posts re . Can you check if I am a good candidate for that?
Me: based on your “virgin” nose photo, I believe kaya nang no implant.
Cge let’s do it.
Let’s convert it to no implant rhinoplasty.
