Yes! COVID-19 vaccination for 12-17 years old with co-morbidities!

OMC process:
For consultation, rates are posted. However, work-ups will be needed for pediatric consultation and will be booked for follow-up and if cleared, certification/clearance will be given.

PS. We do home vaccination for flu and pneumonia babies to seniors.

If our doctors are not your primary doctors (who diagnosed you with that co-morbidity) pls PROVIDE us a copy of your old prescription and/or laboratories.

>For mental health consults:
We cater to diagnosis and symptomatic treatment if needed

***Medical certification is exclusive of consultation fee
Fit-to-work - 500 pesos
Certification for A3 (not our known patient/ new patient) - 500 pesos
Med cert for our patients (known to our facility/previous patients) - 250 pesos

**Dependent on time you will have your consultation

Additional fees:
>Medical certificate issued on the same day after consultation is 250 pesos
>Requesting medical certificate from previous consultation is 350 pesos
>Fit to work must include laboratories, hence, consultation prior is a must. 500 pesos fit to work certification

**SSS and other needed papers should come from your attending physician. We will not sign those papers if you only consulted just for that
