PRE-EMPLOYMENT/ANNUAL MEDICAL EXAMINATION GUIDELINES:

• RECEPTION: Fill up the forms for the medical.

• LABORATORY:
A. BLOOD EXTRACTION: No fasting is needed.

B. SUBMISSION OF FECALYSIS: PEA-SIZED only. Please put your complete details [complete name, age, gender] in a small/sterile container.
Preferred collection place: at Home
*Note: We're not allowing patients to collect on our facility.
Submission time upon collection: within 20-30 mins only.

C. SUBMISSION OF URINALYSIS: collect is midstream clean catch urine at least 30ml in a sterile cup provided.
it must be collected at the Clinic
*For female patients ONLY: Do not collect/submit urine with ongoing menstruation. You are allowed to submit 5 days after your last spotting/menstruation.

• IMAGING:
 Remove necklace or any metal (bra) and wear the patient gown. Patient must follow the instruction of the radiologic technologist. Long hair must be tied above the nape (back of the neck) area.

• PHYSICAL EXAMINATION (Time varies depending on our doctor’s schedule)
 Doctor schedule is at 9am during MON & WED ONLY.
 Doctor schedule is at 11am during TUES-THURS-FRI-SAT ONLY.

8:30 – 10:00AM (MW)
7:30 – 8:00 AM (T-TH-FRI-SAT)

*Bring 1 valid ID, ballpen and refrain wearing long tight sleeves.
