Regional Awarding and Recognition Ceremony for HEALTHY LEARNING INSTITUTION OF SURIGAO DEL NORTE

Healthy Learning Institution (HLI) is a school that actively promotes the physical, mental, emotional, and social well-being of learners and staff. Its rationale is grounded in the idea that health and learning are deeply interconnected.
Healthy Learning Institution ensures that learners are healthy, safe, and supported, because these are essential conditions for effective teaching and meaningful learning.

As part of the award and recognition of these schools is the provision of health, sports and educational materials worth Php 180,000 each schools. While the division office receives Php 800,000 worth of these items. These educational, health and sports items are now shared to different schools of the division.
With this, awards and incentives hopes to give motivation to other schools to make their schools a HEALTHY LEARNING INSTITUION!

Athletes were then referred to Fortaleza Optical Clinic for refraction. Thank you to Dr. Annabeth Navato Fortaleza for the free eye check up and contact lenses of our athletes. 😊

1. Pls fill up the request form for LOA ANNEX B,3 days before your preferred date.
2. Email accomplished form, lab request (as applicable) to:

3. LOA will be sent to your email, print LOA
4. present LOA to hosp or doctor upon consultation

you may also request for LOA at the facility/hospital on the day of your consultation, the HMO staff at the hosp will facilitate the request but waiting time is from 15 -60 min or more.
for emergency cases, directly proceed to ER the hospital staff will facilitate LOA, just inform them you have HMO.
