To all Drug Establishments: drug outlets, distributors and hospitals.
Reminding for the submission of the Sworn Statement as required in the JAO 2 2018.

1. All establishments including hospitals whether VAT or NON VAT are required to submit the sworn statement. For NON Vat establishment kindly attach your BIR Certificate of Registration.

2. You may re-encode the template.
3. In filling up the template we shall follow the EDPMS uploading format.
3.a Wholesalers shall only fill up the wholesaler price column.
3.b Retailers shall only fill up the actual retail price column.
3.c. SRP refers to medicines under MDRP, you may leave it blank if the meds are not under MDRP.
3.d Prices Prior to JANUARY 1, 2019 meaning ur price in December 2018. For ESTABLISHMENTS WITH VAT BEGINNING January 1, 2019 you will deduct 12% from your actual retail price or wholesaler price.
3.e. Only medicines for hypertension, diabetes and cholesterol that you are carrying are the ones that you will declare. There is a FDA list to guide you which medicines are now required to be Vat exempted.
3.f. Indicate the generic name and brand name, if there is no brand kindly indicate the manufacturer name. The brand name or manufacturer's name must be in parenthesis. Dosage strength and form shall be written also.

4. Kindly submit your sworn statement in harcopy. You can drop it in our office or send it through EXPRESS MAIL to:
