ORAL THRUSH OR MILKY TONGUE?

A white patch on the baby's tongue worries many moms as they fear it could be an oral thrush (a fungal infection).

The other week, I had an online patient from abroad whose main complaint was her baby's unresolved oral thrush after a month of treatment. It was a thick white patch that woud not be removed completely after cleaning. The baby was mixed fed for 2 weeks then exclusive breastfeed after. The health worker gave them antifungal meds both for the baby and her nipple's. When I saw the baby's mouth, she has signs of oral restrictions both a lip-tie and a tongue-tie. There was a thin white patch concentrated on the middle of the tongue (left picture). Mom never had any white lesions on the nipple-areola area. Her milk was also cultured and no bacterial or fungal infection was found. Mom just experienced pain during breastfeeding brought about by poor latch due to oral restrictions.

I told the mom that the white patch is just a milky tongue. In a breastfeeding baby, if the latch is good and the baby is suckling effectively, the breast fills the mouth and sweep the milk and dead cells from all areas of the tongue. This baby who has a tongue-tie can't freely do that leaving milk residues in the middle.

A true oral thrush is very unlikely to concentrate on the tongue only but to other parts of the mouth like gums, cheeks and roof of the mouth. The picture on the right was my patient with oral thrush that was followed by diaper candidiasis a week after. You can see that white lesions were apparent in his entire mouth. It resolved after taking antifungal meds.

You can’t biologically separate a baby’s need for milk from their need for comfort. Those night wakings aren’t manipulation, they’re communication. They’re how your baby regulates blood sugar, builds supply, maintains connection, and stays safe.

When we force a baby to “self-soothe,” we’re teaching them silence, not independence. We’re teaching them that no one comes when they call. And when those feeds get skipped, your supply drops, your hormones shift, and your baby’s growth can slow.

Modern culture tells moms that babies should sleep through the night. Biology says otherwise. Babies are meant to wake. They’re meant to feed. They’re meant to need you.

Parehas gapadede pero ang kalahian kay ang unang pares nakatilaw na ug lain gatas ug nakagamit na ug bote ang baby, naa nay struggle sa transition from bottle to breasts, but we still managed to help her baby latch hantud masatisfy ug makatulog.

Samantala ang ikaduha wala jud mi agi ug stress kay ang bata wala katilaw ug lain, kuntento sa dede ug gatas ni mama bisan walay ga agas2. Lami ang paminaw ug walay daghan hilak.

This showed the importance of correct support and information as early as giving birth.

Kaya ug buntis mo karon, i book na inyong breastfeeding session habang wala pa mo nanganak, or better yet, avail our breastfeeding preparation class now.
