🗓 WHEN: March 21, 2026 (Saturday)
🕗 TIME: 8:00 AM – 4:00 PM
📍 WHERE: Imelda Gymnasium, Titay, Zamboanga Sibugay
FREE SERVICES OFFERED:
✅ Eye Check-up
✅ Cataract Screening
✅ Reading Glasses (for qualified beneficiaries)
Note: Double-vision eyeglasses are available for Php 1,000.00 (inclusive of frame and doctor’s fee).
Who can join? All residents of Titay are welcome! We especially encourage senior citizens and those experiencing vision issues to attend.
What to bring: Please bring a valid ID for registration. Arrive early to ensure you are accommodated!

Maayong balita gikan sa **Titay Rural Health Unit**.

Ang mga **YAKAP Beneficiaries nga registered sa Titay RHU** pwede na makadawat ug **Digital Patient ID**.

Kung naa na kay **Digital Patient ID**, dili na kinahanglan moagi sa **Health Benefit Verification**.

📋 **Regular Process:**
Arrival → OPD → Health Benefit Verification → Doctor

⚡ **With Digital Patient ID:**
Arrival → OPD → **Doctor Dayon**

✅ Mas paspas ang check-up
✅ Mas mubo ang pila
✅ Mas sayon ang proseso

Step-by-step guidance on how to register to the PhilHealth YAKAP (Yaman ng Kalusugan Program) using the eGovPH App
📱 1) I-download ug i-install ang eGovPH Super App
Adto sa Google Play Store (Android) o App Store (iOS)
Pangitaa ang “eGovPH Super App”
I-install sa imong phone o tablet
🧑‍💻 2) Mag-create ug account sa eGovPH App
Ablihi ang app
Pilia ang Sign Up / Register
I-enter imong personal info:
Full name
Mobile number (active kay kinahanglan ang OTP)
Email address
Pa-verify ang mobile number gamit OTP (code moabot via SMS)
Pa-verify ang email kung gikinahanglan
Basin pangayoon usab ang imong PhilSys (National ID) ug usa ka selfie para sa identity verification
🧾 3) Sulod sa eGovPH App, pangitaa ang PhilHealth Section
Sa main screen, adto sa NGAs / Services
Pangitaa ang PhilHealth
Pilia ang PhilHealth Member Portal o direktang PhilHealth YAKAP nga link
🏥 4) Mag-register sa YAKAP
Sa sulod sa PhilHealth section, pangitaa ang YAKAP tab
Pilia ang Add YAKAP Clinic (para sa imong kaugalingon o sa dependent)
Pwede nimo i-search ang clinic base sa:
Province
City/Municipality
Pilia ang imong preferred YAKAP-accredited clinic (Titay Rural Health Unit-Main)
I-confirm ang clinic nga imong gipili
— Mao na ni ang imong official na YAKAP provider para sa First Patient Encounter (FPE) ug uban pang serbisyo.
🗒️ 5) Once successful ang registration
✔️ Mosulod ka sa imong YAKAP program
✔️ Pwede ka mo-adto sa chosen clinic para sa:
First Patient Encounter (unang konsultasyon)
Laboratory tests
Prescription sa mga gamot
— ang tanan tanang benepisyo sa YAKAP magsugod human sa FPE.
