PhilHealth YAKAP CARAVAN 2025
Municipal Health Office – Tupi, South Cotabato

The Municipal Health Office of Tupi, in partnership with PhilHealth, proudly presents the PhilHealth YAKAP Caravan 2025 — a two-day health and wellness event for the community!

🗓 When: October 23–24, 2025
🕗 Time: 8:00 AM onwards
📍 Venue: Open Gym, Municipal Compound, Tupi, South Cotabato

🩺 SERVICES OFFERED:
- Medical Check-ups
- Laboratory Services
- PhilHealth Registration

🎯 TARGET PARTICIPANTS:
A maximum of 500 individuals per day will be accommodated:
✅ October 23 -500 participants
✅October 24 -500 participants

📌 Reminders:
- First come, first served basic
B- Bring a valid ID, Marriage certificate, and Birth certificate.
- Open to all residents of Tupi and nearby barangays.
