Releasing ng MCG Cares Card sa mga Pre-Registered na residente ng Barangay Paralayunan

Maraming salamat kay ๐‡๐จ๐ง. ๐‘๐ž๐ฒ ๐๐ข๐ง๐ž๐๐š (Punong Barangay ng Paralayunan) at sa kanyang mga barangay official na nagbigay ng tulong para maging maayos ang pag bibigay ng ating MCG Cares Card

Para sa mga residente ng Brgy. Paralayunan na gustong makakuha ng MCG Cares Card magpasa lamang ng mga sumusunod na requirements at hanapin ni ๐Œ๐ฌ. ๐ƒ๐จ๐ง๐ง๐š๐ฅ๐ฒ๐ง ๐”๐๐ข๐จ๐ง๐  para ma Pre-Register at mabigyan ng schedule.

โœ…Photocopy of Government Issued ID (Na naka Address sa Mabalacat City)
โœ…Photocopy of Vaccination Card
โœ…Photocopy of Booster Card (Kung meron)

Para naman sa mga ibang residente ng Mabalacat City maari kayong lumapit sa inyong mga barangay official at mag request na mag karoon ng pag release ng MCG Cares Card sa inyong barangay.

Nagpunta ang OSAFPH sa Sitio Haduan para magbigay ng karagdagang bakuna laban sa Covid-19 at pag bigay ng QR-Coded MCGCares Vaccination Status Card.

Sinimulan din ang Pilot Testing ng E-Consultant with Digital Repository of Health Data ng ating mga Citizen sa Sitio Haduan. Nagkaroon din ng pamimigay na libreng gamot para sa mga nangangaylangan mula sa Mayor Crisostomo C. Garbo

Natuwa naman ang ating mga kababayan sa Sitio Haduan dahil sa paparating na Programa ni Mabalacat City Mayor Crisostomo C. Garbo.

Pilot Registration sa Sitio Haduan

Ito ang pag rehistro para sa mga Mabalaqueno/Mabalaquena para mag karoon sila ng QR-Coded MCGCares Vaccination Status Card. Ang QR-Coded Card na ito ay maggagamit para sa darating na mga programa ni Mabalacat City Mayor Crisostomo C. Garbo.

Nagunguna na ang Sitio Haduan sa pag rehistro at naging pilot bilang pag hahanda at maging maayos at mapaganda ang programang ito kapag sinubukan na sa mga iba pang barangay ng Mabalacat.

Ang QR-Coded Card na ito ay magagamit sa darating na programa tulad ng mga sumusunod...
