🟣 The Fourth Trimester (0–12 Weeks After Birth)
What Happens to Your Body After Delivery
The first 12 weeks after childbirth are often called the “fourth trimester.”
This is a period of physical healing, hormonal shifts, emotional changes, and adjustment to newborn care.
It’s recovery time — not “bounce back” time.
🟣 Uterus Involution (Uterus Shrinking Back)
After birth, the uterus slowly returns to its pre-pregnancy size.
→ Strong contractions occur in the first few days (afterpains)
→ Uterus usually returns to near-normal size by 6–8 weeks
→ Breastfeeding helps this process by releasing oxytocin
You may feel cramping, especially while nursing — this is normal.
🟣 Lochia (Postpartum Bleeding)
Post-delivery bleeding is called lochia and changes over weeks:
→ Red bleeding (first few days)
→ Brownish discharge
→ Yellow-white discharge
This can last 4–6 weeks or longer.
Important:
→ Flow should gradually decrease
→ Sudden heavy bleeding is NOT normal
🟣 Hormonal Shifts
After delivery:
→ Estrogen and progesterone drop sharply
→ Prolactin rises for milk production
These changes may cause:
→ Mood swings
→ Night sweats
→ Hot flashes
→ Fatigue
→ Dry skin or hair fall
These usually improve over several weeks.
🟣 Pelvic Floor Recovery
Pelvic muscles stretch during pregnancy and birth.
→ Healing may take weeks to months
→ Mild urine leakage is common initially
→ Pelvic heaviness may be felt
Gentle pelvic floor exercises (Kegels) help restore strength.
🟣 Breasts & Lactation
→ Milk usually comes in 2–5 days postpartum
→ Breasts may feel hard or swollen (engorgement)
→ Ni***es may feel tender
Regular feeding or pumping helps relieve engorgement.
🟣 Emotional Changes
Many mothers experience baby blues:
→ Crying spells
→ Anxiety
→ Irritability
→ Feeling overwhelmed
These usually resolve within 2 weeks.
But if sadness lasts longer, it may signal postpartum depression.
🟣 Warning Signs – Seek Medical Help Urgently If You Have:
→ Heavy bleeding (soaking pad every hour)
→ Fever
→ Severe pelvic or abdominal pain
→ Foul-smelling vaginal discharge
→ Chest pain or breathlessness
→ Persistent low mood lasting more than 2 weeks
These symptoms need immediate medical attention.
🟣 How to Support Healing During the Fourth Trimester
→ Rest whenever possible
→ Eat nutritious meals
→ Stay hydrated
→ Accept help from family
→ Move gently
→ Attend postpartum checkups
→ Talk openly about emotional health
Recovery is gradual — be kind to your body.
🟣 Simple Summary
→ Fourth trimester = healing phase after birth
→ Uterus shrinks over 6–8 weeks
→ Bleeding gradually changes color
→ Hormones fluctuate
→ Pelvic floor needs time to recover
→ Emotional ups and downs are common
→ Serious symptoms need medical care
