(Know All Men By These Presents)

That I, we (Student name)_ student of First Year MBBS Class (Year 2012-13) Ameer-ud-Din Medical College, Lahore hereby bind ourselves, our heirs, executors and administrators jointly and severally to the Government of the Punjab for the payment to her for a sum of Rs.100,000/- (Rupees: One Lac only) on demand.

WHEREAS THE ABOVE BONDED (Student name)_ was admitted to the MBBS Class in the Ameer-ud-Din Medical College, Lahore on a clear undertaking by her/ him that He /she would join Government service in the Punjab or A.M.C. and serve for a period not less than three years immediately after the Degree is conferred on his/her.

AND WHERE THE SAME (Student name)_and the said surety (Name of Surety)_ have entered into the Bond in the sum of Rs.100,000/- (Rupees: One Lac only) conditioned for the due performance by (Student name) of the said undertaking.

NOW THE CONDITION of the above written Bond is such that if the said (Student name)_shall join the service of the Govt. of Punjab or A.M.C. or offer himself / herself to join such service of a Medical Officer immediately after the conferment of the MBBS Degree on her/his and shall not quit such service before the completion of the 03 years then the above written Bond shall be null void and of no effect otherwise shall be and remain in full force and virtue.

You have to submit following documents in original and Four attested photocopies of each document with the College Office before depositing the College fee:-



1. Secondary School Certificate (Matric Certificate)
2. Certificate of the O-Level.
3. Equivalence of the O-Level
4. Certificate/ detailed marks certificate of F.Sc (Pre-Medical)
5. Certificate of the A-Level.
6. Equivalence of the A-Level
7. Copy of the Entry Test Result
8. Provisional Certificate/ Character of College last attended.
9. Domicile Certificate
10. Board Migration Certificate in case of the F.Sc other than B.I.S.E. Lahore,
11. Five latest passport size photographs (sky blue background) duly attested (back-side) by a Government Officer (Grade-17 or above).
12. Certificate of Tetnus Hepatitis “B”.
13. Father / Guardian’s I.D. Card copy.
14. Surety Bond (specimen and) on a stamp paper of wroth Rs.100/- duly attested by the Civil Judge.

just called QAMC.. They said it'll take 1 or 2 more days to issue call letters..if students do not receive there call letters by 4th 0r 5th December, they should immidiately contact QAMC.
