LEAP for Intersex and Trans This is a passion project, to raise correct awareness about Intersex and Transgender individuals. The baby has no say in getting a sex assigned at birth!

When a baby is born it is assigned a sex which is either male (having Male anatomy), female (having Female anatomy) or Intersex ( ambiguous genitals). When the baby grows up, 2 years onwards he/she starts expressing their choices in what they like to wear, what role they play in a game, what clothes they want to put on, etc, that's how they express their gender the way they perceive themselves. In most cases a baby assigned as male child, expresses traits of masculinity, that is also what is expected of him and a female child expresses traits of femininity. Unfortunately in Pakistan most cases of ambiguous genitals are misunderstood, due to lack of knowledge, are frowned upon by parents, siblings and peers, and either given up to strangers or abused at home. Ideally the intersex child should be sent home with a sex assigned at birth according to the results of certain tests, physical appearance and future sexual potential, in accordance with the latest guidelines. And the parents should be told to allow free expression of gender to understand how they perceive themselves. Only some of those children have dysphoria. A dysphoric individual will be in extreme distress and depression because of the sex assigned at birth and his/her perception of himself/herself and they will need help transitioning. So a Trans man would be a child with female anatomy but a masculinized brain and a Trans woman would be a child with a male anatomy but a feminine brain. It isn't an easy process and because of the discrimination and stigma they suffer from mental health issues. So in short Man = Trans man & Woman = Trans woman when dealing with human rights. Health needs for trans are various too. There are guidelines available on that. Please be advised that Islam is a tolerant religion, it allows surgeries in certain cases (Al-Azhar University of Egypt has given fatwas back in 1989)
Sex assigned at birth is Not the same as Gender. Gender is a spectrum and all of us experience life with a mix of femininity and masculinity, easily explained by the hormonal balance or imbalances in us. Once that child reaches puberty, certain hormones kick in, and the adolescent now experiences sexuality too. That's when sexual orientation comes into play. Your brain tells you who you desire physically, you may not express those desires and control them at will and all individuals have sexual and reproductive health rights that should be known to all healthcare providers. Lack of knowledge is no excuse! PMDC has set code of ethics on unbiased practice! Those codes are shared on this page too.

If your teenager says,
“Ammi… Abbu… I don’t feel comfortable in my body,”
that is not rebellion.
That is vulnerability.

They are not trying to embarrass you.
They are trying to trust you.

Adolescence is already a storm.
Gender dysphoria makes it feel like drowning inside that storm.

When parents respond with anger, shame, or rejection…
the child doesn’t “become normal.”

They become alone.

And in Pakistan, we know what happens to children who are alone.
Many end up on the streets.
Begging.
Exploited.
Forced into unsafe environments.
Not because they wanted that life.
But because home stopped being safe.

Research across cultures shows that family acceptance reduces depression, self-harm, substance abuse, and su***de risk dramatically.
Support doesn’t “create” gender diversity.
It protects your child from harm.

You don’t have to understand everything in one day.
You don’t have to have all the answers.

But you do have to keep the door open.

Say this instead:
“I don’t fully understand yet… but I love you. Let’s learn together.”

That sentence can save a life.
Your child needs a parent.
Not a judge.
Not a society spokesperson.

If you’re confused, scared, or overwhelmed — get guidance.
Don’t give up on your child.

Because when families hold their children… the world becomes safer for them.

Plagued by cultural expectations, many among us remain undiagnosed, and the majority of healthcare providers remain unaware of the healthcare that they need.

Every child, no matter what genetic makeup they have, no matter what bodyparts they are born with, and no matter how different they are, deserve love and acceptance from their parents and family.

Yes it's tough!
Yes it's a challenge!
But rejecting your own child, shaming them for being different, isolating them, and abusing them or even thinking they need fixing which doesn't align with their identity is the failure of our society!

Parents who come forward to get proper counseling regarding their inters*x or transgender child are shifting the norms for good!

Everyone is answerable to Allah for the way they behaved with others!

Please unlearn the hate and relearn support!

You won't find me smiling and laughing all the time.
Life is very real and your good work gets challenged often.
My work doesn't break families, instead it saves at least one person inside the family from emotional exile.
Social isolation is a slow invisible wound. It doesn't bleed but it kills hope, belonging and sometimes even life.

My work is sacred.
In Islam, mercy is not conditional on comfort. Our Prophet (s.a.w) was challenged and complained about too.
Truth always unsettles systems built on fear.

Through my work I offer;
Dignity instead of shame
Dialogue instead of abandonment
Compassion instead of punishment

Inna Ma'al Usri Yusra
With hardship comes ease
Not after. With. Together

Even when my heart feels heavy Allah has placed relief somewhere in my path.
When I serve a vulnerable minority with honesty and ethics, I think of it as Ibadah.
Quiet, unseen, powerful worship.
Planting ease in my client's life.
I'm divinely guided because Allah doesn't choose random people for difficult work.
He chooses hearts that can carry it without being cruel.

If you treat someone's identity as a mental illness you're practicing outdated, unethical medicine.
If you deny therapy that improves bone health, mood and quality of life, you are causing harm.
Doctors who do not update their knowledge are not neutral, they are dangerous!

Inters*x and transgender care isn't just about hormones and anatomy, it's about listening, respecting and learning!

Lets talk about another important inters*x variation known to doctors as Aromatase Deficiency.

In simple words, the body struggles to concert testosterone to estrogen.

In babies with 46XX the ge***al may look more masculine than expected.
Cl****is maybe larger.
L***a may look more fused.
In babies with 46XY the ge****ls may look typically male just under masculinized depending on severity.

Individuals with 46XX will have a uterus, ovaries and fallopian tubes but low levels of estrogen and higher testosterone.

Individuals with 46XY will have te**es and high testosterone levels but no uterus, ovaries, and perhaps metabolic issues later in life.

Some signs untrained professionals miss easily are:
👉 Minimal or no breast development
👉 No periods or irregular cycle
👉 Deep voice
👉 Increased facial and body hair
👉 Muscular build
👉 Persistent fatigue
👉 Emotional volatility from hormone imbalance.
👉 Acne
👉 Reduced fertility
And maybe insulin resistance later too

Some people with this variation especially those with 46XX may experience gender incongruence and need affirming care.
We need to be mindful of the intersectionality.

These individuals deserve understanding, timely diagnosis, HRT and psychological safety and support.

Awareness leads to compassion and I invite you to use your compassion for offering inclusive care.

If you are a healthcare provider please ensure 5 things in your practice that are an ethical consideration.

1. Consent - every test every procedure needs to be fully explained so that the client can make an informed decision.

2. Protect Confidentiality - please remember who is your client. Divulging information to parents, guardians and loved ones is unethical!

3. Delay unnecessary medical interventions. If they are not life threatening then they are not urgent.

4. Your language matters. Instead of using complex medical terms, let your compassion come out in simple kind words. Instead of making them feel like walking diseases, make your practice client centered.

5. Become more inclusive and connect with other gender affirming and inters*x affirming care providers - make multidisciplinary teams that are trained in inters*x and transgender healthcare.

LEAP for Inters*x rejects unnecessary cosmetic procedures and conversion therapies on inters*x and transgender persons and encourages surgeons to #stopIGM , instead offer gender affirming services. Lack of knowledge is no excuse! PMDC has set code of ethics on unbiased practice! Those codes are shared on this page too.
