LEAP for Intersex and Trans This is a passion project, to raise correct awareness about Intersex and Transgender individuals. The baby has no say in getting a sex assigned at birth!

When a baby is born it is assigned a sex which is either male (having Male anatomy), female (having Female anatomy) or Intersex ( ambiguous genitals). When the baby grows up, 2 years onwards he/she starts expressing their choices in what they like to wear, what role they play in a game, what clothes they want to put on, etc, that's how they express their gender the way they perceive themselves. In most cases a baby assigned as male child, expresses traits of masculinity, that is also what is expected of him and a female child expresses traits of femininity. Unfortunately in Pakistan most cases of ambiguous genitals are misunderstood, due to lack of knowledge, are frowned upon by parents, siblings and peers, and either given up to strangers or abused at home. Ideally the intersex child should be sent home with a sex assigned at birth according to the results of certain tests, physical appearance and future sexual potential, in accordance with the latest guidelines. And the parents should be told to allow free expression of gender to understand how they perceive themselves. Only some of those children have dysphoria. A dysphoric individual will be in extreme distress and depression because of the sex assigned at birth and his/her perception of himself/herself and they will need help transitioning. So a Trans man would be a child with female anatomy but a masculinized brain and a Trans woman would be a child with a male anatomy but a feminine brain. It isn't an easy process and because of the discrimination and stigma they suffer from mental health issues. So in short Man = Trans man & Woman = Trans woman when dealing with human rights. Health needs for trans are various too. There are guidelines available on that. Please be advised that Islam is a tolerant religion, it allows surgeries in certain cases (Al-Azhar University of Egypt has given fatwas back in 1989)
Sex assigned at birth is Not the same as Gender. Gender is a spectrum and all of us experience life with a mix of femininity and masculinity, easily explained by the hormonal balance or imbalances in us. Once that child reaches puberty, certain hormones kick in, and the adolescent now experiences sexuality too. That's when sexual orientation comes into play. Your brain tells you who you desire physically, you may not express those desires and control them at will and all individuals have sexual and reproductive health rights that should be known to all healthcare providers. Lack of knowledge is no excuse! PMDC has set code of ethics on unbiased practice! Those codes are shared on this page too.

Let's talk about a fascinating biological phenomenon...
Where two embryos become one fetus during early pregnancy!

Chimerism

This is when an individual is born with two different set of genetic materials.
They may have 46XX and 46 XY simultaneously, apart from the way their body may look, or the way they experience puberty...

They may be born with streak go**ds, ovote**es, ovarian and testicular tissue on either or both sides...
Nature will express itself, not as a disease with as variation...
They may have reproductive concerns when they grow and develop differently from typical male and female bodies.
Many Chimera will have typical bodies and no confusion related to their gender.

Since genetic tests are too expensive, and they confirm what the person is saying to be their truth, we offer affirming care to support their overall wellbeing and not fix them in typical binary boxes.

They may experience gender incongruence as well and due to stigma they may not seek help.

Affirming care saves lives and changes lives for better!

3 Things You Can Do To Protect Gender Variant and Bodily Diverse individuals

1. When someone mocks, generalizes or blames the entire community, stop them! Your one sentence could save someone else from a lifetime of shame!

2. Transgender and inters*x identities are not new or western.
They have existed in our culture for centuries. Understanding reduces fear and fearlessness reduces violence!

3. Treat gender minorities the way you would want your own child/siblings to be treated:
With dignity
With empathy
With humanity

Every kind interaction becomes a ripple of safety.

Instead of shaming people about their inability to conceive, thinking they need to be fixed so they can reproduce, undermining that the Qur'an says it is Allah's will who He keeps barren, raises alarming concerns about the bodily integrity, autonomy, and liberty to express who they truly are in terms of their experienced gender, I urge you to learn more about diversity in humans with a scientific lens!

Those who are spreading misinformation and disinformation, don't even know if they themselves have an average chromosome combination, hormones and body parts or not...

What destroys relationships is the inability to accept the fact that Allah creates and all humans need healthcare as per their need as their basic right!

What destroys relationships is also enforcing what the majority thinks is right even if it means human rights violation.

Relationship dynamic improve when you accept people for how they are, for the talent or skills they have and for the kindness and affection you can show despite challenges!

Let's talk about the Most common 46XX inters*x variation in Pakistan - Congenital Adrenal Hyperplasia or CAH!
A natural variation where the adrenal glands make too much androgens and not enough cortisol or aldosterone.

What you can notice are ge****ls that may be different from what families expect for a girl:
A bigger cl****is and/or Partially fused l***a.
These individuals do have what is typical reproductive structure for females...
A uterus
Ovaries

But here are the medical symptoms people miss...
Low energy or constant fatigue
Dizziness or feeling faint
Episodes of vomiting
Frequent diarrhoea often labeled as IBS or gut issue
Dehydration in warm weather
Salt cravings
Low BP
Darkening of skin creases.

And these are happening because the adrenals glands are working extra hard!

Most 46XX individuals with CAH identify as girls or women.
And a small percentage feel gender incongruence and may need to transition.

Inters*x variations and transgender identities overlap often and these individuals deserve the care that sees all of them not just one part of their biology!

Let's learn a bit about the most common 46XY variation in Pakistan.

Androgen insensitivity syndrome or AIS
In simple words - the body has XY chromosomes but the cells don't respond to testosterone.

For most babies with complete AIS, the ge****ls appear like a typical baby girl. That means it's easy for parents and doctors to realize the baby will have XY chromosomes.

For partial AIS appearance varies.
Some babies will have slightly large cl****is, a small p***s or partially fused l***a, etc

Inside the body, these individuals usually have...
Internal te**es (undescended or in the abdomen or groin)
No uterus
No fallopian tubes
No periods

And since body doesn't respond to testosterone, they have feminine body shape, feminine brain, and excess testosterone may convert to estrogen.

Common symptoms people ignore for AIS individuals include, no periods, short vaginal canal, minimal body hair, hernias or swollen groin having te**es, difficulty inserting tampons, and discomfort in abdomen or constipation...
All of this is biology, not a disease or defect.

Most and not all complete AIS individuals identify as women.
Those with partial AIS may experience gender incongruence.

AIS is at crossroads because for some the chromosomes, hormones and identity may not align neatly.

They need understanding, privacy and affirming healthcare.

Inters*x is an umbrella term for natural variations in chromosomes, hormones, or anatomy that don't fit the typical male or female definitions.
In Pakistan the most common inters*x variations are:
Androgen insensitivity syndrome for 46XY
And Congenital Adrenal Hyperplasia for 46XX individuals.
For long they have been thought as diseases, they are not!
They are not new.
They are biology's diversity.

Please use the terms Inters*x variations instead of anomaly, abnormality, disorder for anyone with these differences.

Building understanding together!
I have come back from a 2 day summit, a first of its kind, where I met inters*x advocates from 9 countries, that included doctors, medical professionals and inters*x human rights activists.
Representing my country, I highlighted how many people don't know they could be inters*x as they identify as men and women and khwajasira.
Since inters*x and transgender healthcare isn't taught separately in our country, even highly specialized professionals aren't fully aware about the 40+ medically recognized inters*x variations.

All participants have successfully drafted the first healthcare blueprint, which will be an evolving document.

I have taken it upon myself to raise awareness about such individuals and design workshops/seminars to disseminate affirming care knowledge among our local healthcare providers.

Stay tuned for more posts/reels as I help healthcare providers, students, and the public understand that s*x diversity is natural, language matters, and affirmation begins with understanding.

Imagine having a medical condition but being treated like a moral problem...
One of my clients is a 22 years old inters*x woman.
She has hypopituitary hypogonadism - her body doesn't produce reproductive hormones.
She never had regular periods, had bone aches and early osteopenia.
Estrogen therapy has helped a lot.
But now that she is married, her husband is asking her to stop estrogen and get pregnant!
Her psychiatrist called her "anxious" suggesting anti-anxiety medicine, instead of listening to why she's anxious...
She doesn't need more pills. She needs the right kind of support...
She needs to consent.
She needs autonomy for her body.
She deserves to be safe in her body!

Inters*x people are not disorders to fix, but humans to honour!
Let's start doing that.
