A 6-year-old girl is being evaluated 5 days after she suffered a severe traumatic brain injury. She underwent decompressive craniectomy and placement of an external ventricular drain, and has been receiving mechanical ventilation. Elevated intracranial pressures have been treated with sedation, mannitol, maintenance of normocarbia, and cerebrospinal fluid drainage. She has been tolerating nasogastric tube feeds of a standard pediatric formula at 50 mL/hour. Her weight is 20 kg. On physical examination, the girl is intubated and sedated. She does not open her eyes spontaneously and withdraws all extremities equally upon pain stimuli. Pupils are 4 mm, equal, and sluggishly reactive. Her mucous membranes are moist, and she has periorbital and soft tissue edema. Urine output over the past 24 hours was 240 mL.
