These are the radiograph of a 6 year old boy with a "shepherd crook deformity" of his left femur(which occurred due to multiple fractures of his left femur due to him being a patient of osteogenesis imperfecta). An osteotomy was done and his bone straightened and a 2.5mm elastic nail was inserted into his very narrow medulla canal.

Osteogenesis imperfect also known as brittle bone disease is one of commonest genetic bone diseases. The incidence is one in 20000 live - births. The problem lies in the abnormal synthesis of type 1 collagen which causes abnormalities of teeth, ligaments, bones, skin and sclera. Clinically it presents as blue sclera, lax ligament, frequently fracturing bones and weak teeth. There are various types of the disease with varied spectrum of disease presentation. No medical treatment is presently on offer . Chiefly prevention and management of fractures and correction of post-fracture-healing deformities is the mainstay of treatment helping the patients to lead a close to normal life.

CPT or congenital pseudarthrosis of tibia is a rare and extremely difficult condition to treat. Here the child has a comparitvely short limb and the tibia( leg bone) is curved anteriorly.A fracture usually takes place by the age of two years. No cause is known but there is an association with neurofibromatosis .Surgery is the only solution but is less successful if done too early in life. The child is allowed to walk in a shell- like brace so that he grows without having a fracture and his chances of success after surgery are improved. There are various options of surgery which means that none of them is ideal but are worth an effort.
