A young boy with complaint of generalized abdominal pain from last 4 days .History examination and radiological finding suggestive of perforated appendix with mild free fluid in pelvis.laprscopy done about 500 ml pus in abdomen with appendix perforated at tip. Appendectomy done with abdominal lavage . Alhamdolillah pt fine on follow up visit .May Allah bless the pt complete recovery .ameen.

Maintenance of blood supply to distal part until proper expertise or to get the time to save the distal part from ischemia blood supply can be maintained with help of simple drain connecting both proximal and distal part.A young boy with RTA injury to upper limb muscles nerves and brachial artery cut. Artery repaired with reverse great saphenous vein graft median nerve also repaired bone was intact.before repair blood supply was restored through drain to prevent ischemia then reverse vein graft applied .May Allah bless pt with complete recovery.ameen.

Difficult case of laproscopic cholecystectomy of pt from dubai for empyema gall bladder at same time pt had pancreatitis managed for pancreatitis when enzymes settled lap chole done in same admission . Interesting case importance of on table cholangiogram and wide good Drainge
Pt history of open chole at periphery developed biliary peritonitis was explored at tertiary care hospital abdominal.lavage was done with wide drain pt became stable with drain .bile was coming from drain MRCP done showed cut of CBD .Gastroenterologist opinion sough advised to go for surgery .Hepaticojejunostomy was planned pt explored .on exploration it was found cut end is cystic duct stump and cbd is intact.On table cholangiogram performed which showed intact cbd contrast going into gut .stump.was closed.drain kept removed after 14 days .lfts normal drain output nil.So it is lesson some time c d shows cut due to some pressure on it .t tube cholangiogram should be used routinely in difficult and doubtful cases

Female pt pain on off pian rt iliac fosaa.severe tenderness and ultrasonic finding of acute appendicitis .diagnostic laprscopy was planned on laprscopy there was omentum adherent to uterus and anterior abdominal wall acutely inflamed appendix and caecum adherent to abdominal wall forming a mass with omentum and rt fallopian tube fallopian tube was swollen .Adhenolysis done appendectomy done .fallopian tube tube released from caecum.which also released from abdominal
Wall. Alhamdolillah pt is fine on follow up.
Pt with pain rt ilac fossa has too much differentials.so.it is always best to go laproscopic approach than open approach for appendectomy so not to miss the other things .
