We just witnessed a miracle at our institute (Cardiac Family- Fauji Foundation Hospital Peshawar Pakistan)
It was a normal cold day(30th July 2025) at our institute when suddenly Code activated by Pervez Abbasi- Coordinator Cardiology and Cardiac Surgery. that we will be receiving this patient shortly and need
Cardiac surgeon,
Cardiac Anesthetist,
Cardiac Perfusionist,
And
Intervention Cardiologist
ASAP in CATH Lab.
39 yrs healthy male pt with no known major comorbids received on ventilator from periphery,
multiple VT runs, Cardiac arrest, Cardiogenic Shock,
3 times CPR performed on way and primary receiving centre,
Pt shifted directly to Cathlab from Ambulance 🚑
VT run and arrest- 10 min CPR and DC cardioversion while on Cath table,
LAD prox total occlusion noted on Angio Shot,
rescue PCI to LAD performed by Prof Dr Asif Nadeem, while assisted by Dr Naveed Registrar Cardiologist and Cath Tech Saeed.
severely impaired LV- EF 20,
Pt shifted to Cardiac-ICU,
pt on very high inotropic supports,
I decided to put an IABP/ Intra Aortic Balloon Pump through Rt Femoral approach,
pt improved,
extubated safely by Anesthetist and Intensivist Dr IkramUllah Khan,
IABP Removed once inotropic supports demand decreased.
pt mobilized and discharged to the concerned hospital connected with the primary hospital.

Angio pic,
monitor after PCI,
IABP screen shared

Congenital Heart Diseases.
A short overview.
عموما دل میں سوراخ پیدائشی نقص Congenital Heart Disease ہوتا ہے (ایک قسم Ventricular Septal Rupture دل کے دورے کی وجہ سے بنتی ہے).
دل کے سوراخ کو سمجھنے کیلیئے یہ جاننا ضروری ہے کہ دل میں چار چیمبر ہوتے ہیں،
دو Atrium اور دو ہی Ventricles.
دونوں Atrium اور Ventricles کے درمیان ایک دیوار ہوتی ہے جسے Atrial Septum اور Ventricular Septum کہا جاتا ہے۔
Right Atrium
دوبڑی رگوں Superior Vena Cava اور Inferior Vena Cava کے ذریعے جسم سے خون وصول کرکے
Right Ventricle
کو بھیجتا ہے جو کہ آگے پھیپھڑوں سے آکسیجن لینے کیلیئے خون Pulmonary Artery میں پمپ کرتا ہے،
پھر پھیپھڑوں سے آکسیجن سے بھرپور خون چار Pulmonary Veins کے ذریعے واپس دل میں
Left Atrium
میں آتا ہے اور اس سے
Left Ventricle
کہ طرف جاتا ہے جو کہ آگے پورے جسم کو "شہہ رگ" Aorta کے ذریعے خون کی سپلائ کرتا ہے۔
اب اصولاً Right Atrium اور Left Atrium کے درمیان ایک دیوار یعنی Atrial Septum ہونا چاہیے، اگر اس دیوار میں کوئ سوراخ ہوگا تو اسے
Atrial Septal Defect- ASD
کہا جائے گا۔
دل میں سوراخ ہونے سے دل کے چیمبرز کا خون ایک دوسرے کے ساتھ مکس ہوگا اور مریض اس وجہ سے تکلیف میں مبتلا ہوگا- عام طور پر علامات میں مندرجہ ذیل چیزیں دیکھی گئی ہیں؛ معمولی کھیل کود میں دل کا تیز تیز دھڑکنا یا پھر آنکھوں کے آگے اندھیرا چھاجانا، عمر کے ساتھ وزن کا نا بڑھنا، اکثر اوقات سینہ خراب اور انفیکشن رہنا، اور دل کی دھڑکن کا ردھم خراب ہونا Arrhythmias۔
Atrial Septal Defect- ASD
کی پھر آگے مزید پانچ اقسام ہیں؛
1: Patent foramen ovale- PFO
(Foramen ovale)
جو کہ ایک نارمل بچے میں ہوتا ہے اور پیدائش کے وقت جب بچہ اپنا سانس لینا شروع کرتا ہے تو خود بخود بند ہوجاتا ہے، اگر بند نا ہو تو اسے PFO کہتے ہیں، اس کا علاج نسبتاً آسان ہے۔
2: Ostium secundum- ASD Secundum
بچوں میں دل کا سب سے سے زیادہ پائے جانے وال سوراخ یہی ہے، اور ہمارے ملک میں لوگوں کو آگہی نا ہونے کی وجہ سے اکثر ڈائگنوسس بہت دیر سے ہوتی ہے، اس کا علاج دو طریقوں سے ممکن ہے-
)Device Closure-(
انجیو گرافی کی طرح کا ایک پروسیجر ہے جس میں ایک ڈیوائس کے ذریعے دل کے سوراخ کو بند کردیا جاتا یے اور اکثر عوام اسے لیزر یا کیمرے والا آپریشن سمجھتی ہے، اگر سوراخ بڑا ہو تو تب اس کو اوپن ہارٹ سرجری کے ذریعے بند کیا جاتا ہے۔
3: Ostium primum- ASD Primum
Septum primum
کی مکمل گروتھ نا ہونے کی وجہ سے بنتا ہے اور اس کا علاج صرف اوپن ہارٹ سرجری یعنی آپریشن سے ہی ممکن ہے۔
4: Sinus venosus ASD
Inferior and Superior Vena Cava
اور Fossa Ovalis کی وجہ سے اس کی آگے پھر دو مزید قسمیں ہوتی ہیں اور ان کا علاج بھی صرف اوپن ہارٹ سرجری یعنی آپریشن کے ذریعے سے ہی ممکن ہے۔
5: Coronary sinus ASD
دل کو خود بھی زندہ رہنے اور کام کیلیئے کچھ رگوں کے ذریعے خون سپلائ ہوتا ہے اور کچھ رگیں دل سے خون واپس Right Atrium میں لے کر آتی ہیں جنہیں Coronary Sinus کہا جاتا ہے، Coronary Sinus کی ڈیویلپمنٹ ٹھیک نا ہونے کی وجہ سے جو سوراخ بنتا ہے اسے Coronary Sinus ASD کہتے ہیں اور اس کا علاج بھی صرف اوپن ہارٹ سرجری یعنی آپریشن کے ذریعے سے ہی ممکن ہے۔
Subtype; Fenestrated Inter Atrial Septum - Multiple ASDs:
اس میں دونوں چیمبرز کے درمیان دیوار میں چھوٹے سائز کی ایک سے زائد سواراخیں ہوتی ہیں، عموما ڈائگنوس ہونے پر ابزرو کرتے ہیں اور پھر پیڈز کارڈیالوجسٹ کے مشورے سے ڈیوائس کلوزر یا سرجیکل ریپیئر کی جاتی ہے۔

دل کا سوراخ ASD اگر 5mm سے کم ہو تو اس کو چھیڑا نہی جاتا اور مریض کو صرف ہر چند ماہ بعد ECHO اور دوسرے ٹیسٹوں کے ذریعے ابزرو کیا جاتا ہے،
اس کے علاوہ صرف
PFO اور ASD Secundum
کا علاج Device Closure یا لیزر و کیمرے والے آپریشن سے ممکن ہے، باقی اقسام کے سوراخ اوپن ہارٹ سرجری سے ہی ممکن ہیں۔,._.,.
اب آتے ہی Ventricular Septal Defect-VSD کی طرف،
Right Ventricle اور Left Ventricle کے درمیان عموماً ایک دیوار ہوتی ہے جسے Ventricular Septum
کہا جاتا ہے، اس میں سوراخ ہونے کی صورت میں اسے
VSD
کہتے ہیں۔
Ventricular Septal Defect- VSD
کی چار اقسام ہوتی ہیں۔
Type 1: (infundibular, outlet- Supracristal VSD)
یہ اکثر Aortic Valve یا Pulmonic Valve کے قریب پایا جاتا ہے، اور اس علاج اوپن ہارٹ سرجری یعنی آپریشن کے ذریعے کیا جاتا ہے۔
Type 2: (membranous, Perimembranous)
یہ سب سے زیادہ پائ جانے والی قسم ہے اور والوز سے نیچے ہی پایا جاتا ہے، بسا اوقات چھوٹے سائز کی صورت میں خود بخود بند ہونے کا چانس بھی ہوتا کے اور اس کا علاج دو طریقوں سے ممکن ہے-

، اگر سوراخ بڑا ہو تو تب اس کو اوپن ہارٹ سرجری کے ذریعے بند کیا جاتا ہے۔
Type 3: (inlet or atrioventricular canal defect)
یہ قسم Mitral Valve یا Tricuspid Valve کے نسبتا قریب ہوتی ہے، علاج صرف اوپن ہارٹ سرجری یعنی آپریشن سے ہی ممکن ہے۔
Type 4: (muscular, trabecular)
یہ Ventricular Septum کے مسلز والے حصے میں ہوتا ہے اور عموماً خود بخود بند ہونے کا کافی چانس ہوتا ہے۔
