🔻❗️. Only 2 MOs placed in Night shift to cater 150+ admitted patients in night which is not possible nor does justify HR Rationalisation at DHQ . MS advised to pull MO/WMOs from OPDs (Filter Clinic) is sufficient in both morning & Evening Shifts OPD. Consultants shall run their OPDs. 🔴🔴🔴

🟢Clinical & Nursing care counter verified by notes and pts feedback. Which remained satisfactory.

Detail inspection of *DHQ RAJANPUR*

Staff Presence is satisfactory , Rationale and justified utilisation has been ensured now.

Dress code is implemented.

*CheckList* formulated for DMSs , and they are advised to ensure reporting of Trauma within 5 Minutes .

Triage Protocols implemented.

📍Thalasemia Unit spot identified and MS directed to ensure
📍CSSD is partially implemented , all SOPs discussed and MS asked for 7 days to make it completely fucntional as per criteria (Separate Entry , Exit , Dirty Area , Packing area , Sterile instrument etc).
🟢OT Swabs checked and found to be negative (Last 2 Months).

🟢No Biomedical Equipment in Stores now.

🔴Nurses have no knowledge regarding operational SOPs of Burn Unit , Dr Khubaib has been assigned to conduct 2 days refresher on Parkland and Burn Care.

*ONCOLOGY OPD* has been initiated at DHQ RP, undersigned has made a 4 bedded Onco Unit adjacent of ICU. A multidisciplinary roster has been formulated for admitted patients to get them best possible palliative care.

MS has been asked to implement 1 attendant policy in true letter and spirit.
📍MS directed to submit Evening and Night Surgeries by each department. It is advised if any patient found referred without any justification
It ll have consequences for both administration and clinical staff.

Few area in-charges didn’t conducted *TEAM HAND SHAKE* at their borders with other UCs / Areas which can POP UP as missed area during campaign.
DDHO Advised to monitor the activity tomorrow.

All staff adhered with dress code except few , MS required to ensure *DRESS CODE* with all staff.
➡️ MS directed to establish Lab & X Ray in Emergency Unit.
➡️Trauma Center adjacent to ER is also working which is not appropriate : MS asked to shift all such focus on ER , and trauma shall be utilised as OT Complex in all shifts along with surgical & Allied ward .
🟢Dialysis Functional , Feedback +.
🟢Gynae Labour Room Functional.
✳️Main Medicine store is established in 1 Room. Pharmacisit and Pharmacy Managers directed to *AUDIT / IMPLEMENT* directions conveyed by competent authority regarding Sub-stores / good practices .

Visited RHC Fazilpur and conducted field visits in scattered areas of Fazilpur to review healthcare service delivery and ongoing field activities.
🔹 RHC Fazilpur: Facility found functional with availability of staff and essential services.
🔹 OPD & Patient Care: Smooth service delivery and patient handling observed.
🔹 Pharmacy: Availability of essential medicines ensured with proper stock management.
🔹 Field Activities: Monitoring of outreach services in scattered areas, including community-level healthcare and family planning services.
🔹 Staff Performance: Field staff found active and engaged in service delivery.
🔹 Record Keeping: Documentation and reporting found up to date.
Overall, both facility-based and field operations were found satisfactory, with directions issued for further improvement in outreach and service quality.
