This is a perfect example of a tooth that can be treated in two different ways: with an overlay or a crown.

In this case, performing an overlay is straightforward, but what about the remaining tooth structure—will it be sufficient to protect the tooth from fracture?

And what about the exposed root surface on the vestibular side? Would you cover it with an overlay or with composite?
With a vertically prepared TT crown, I can preserve as much tooth structure as possible while addressing all these issues.

Overlay is often seen as a more conservative alternative to a crown. But does „less invasive” always mean fewer complications ? What is your own experience ?

1️⃣ Always start with Class V cavities.
It improves isolation and makes the entire procedure much easier.
2️⃣ Teflon isolation is often more than enough.
A properly placed Teflon cord simplifies both isolation and retraction.
3️⃣ Pay attention to the shape factor.
Always strive for a positive cavity shape — it directly guides your matrix selection.
4️⃣ Limited access?
👉 Always use a transparent matrix.
It allows proper light curing of the composite.
5️⃣ Teflon floss = the easiest and most efficient way to adapt the matrix.
If you haven’t used it yet — try it.
6️⃣ There is only one ring that works on the mesial of the first premolar:
👉 TOR MD ring.
7️⃣ Remember:
🔹 The ring is for separation
🔹 Teflon floss is for matrix adaptation
8️⃣ Check the MSS – Matrix Selection System.
Matrix thickness, height, and curvature are essential for building a proper contact point.
