Persistent pneumothorax following many months of right pneumonectomy should always be suspicious for air leak usually from brocnho-pleural fistula.
To my surprise, this was secondary to esophago-pleural fistula confirmed by barium study.This was completely sealed by surgery.

In a trauma patient and if CXR shows unusual elevation of the diaphragm, an important differential diagnosis not to be forgotten is acute diaphragmatic rupture. On CT it is important to review coronal and sagittal images . In this example the stomach has herniated into the left hemithorax giving the collar ( Hour-glass) sign.
