FAQ Series: Part 1

Think of your vascular system like a plumbing network: when everything flows smoothly, life runs without a hitch.

But just as a leaky pipe can disrupt water pressure, a venous leak in erectile dysfunction (ED) causes blood to drain too quickly from the p***s, preventing a firm er****on. In a healthy system, veins act like valves, maintaining pressure to sustain blood flow where it’s needed. With venous leak ED, these 'valves' malfunction, leading to frustration and challenges. Understanding this 'plumbing problem' is the first step toward finding the right fix!

Q) I’ve heard that 80% of erectile dysfunction cases are caused by venous leaks. How can I determine if my ED is due to a venous leak?

A) The most reliable way to diagnose a venous leak is through a Pe**le Doppler Ultrasound. This non-invasive test uses sound waves to create detailed images of blood flow in the p***s. Here’s how it works:

• Resting State: Your blood flow is assessed while you’re not aroused.
• Er****on Induction: A medication (such as alprostadil) is administered to stimulate an er****on, allowing the specialist to evaluate blood flow and identify any leakage.

This test helps pinpoint whether a venous leak is the cause of your ED, guiding the next steps toward effective treatment.

At VIC, our vascular surgeons are like specialist plumbers, expertly diagnosing and repairing these leaks to restore proper function. Contact us at +65 6694 6270 to book a consultation with our specialists for proper evaluation and treatment.

Deep vein thrombosis (DVT) is a medical condition characterized by the formation of a blood clot (thrombus) in a deep vein, usually in the legs.

Known often as economy class syndrome as it refers to the risk of DVT during long periods of immobility such as long haul flights in economy class seating.

Why Does It Occur During Long Flights?
1. Immobility: Sitting for extended periods reduces blood flow in the legs, increasing the risk of clot formation.
2. Compression: Tight seating arrangements can compress veins, further inhibiting blood flow.
3. Dehydration: The low humidity in airplane cabins can lead to dehydration, which can thicken the blood.
4. Low Cabin Pressure: Changes in pressure can affect circulation.

Prevention Strategies
1. Stay Active: Move around the cabin periodically (at least every hour).
2. Leg Exercises: Perform ankle pumps and calf raises while seated.
3. Wear Compression Stockings: Graduated compression stockings can help improve circulation.
4. Stay Hydrated: Drink plenty of water during the flight.
5. Avoid Alcohol and Caffeine: These can contribute to dehydration.

Plaque buildup in the carotid arteries can lead to:
- Reduced blood flow to the brain, increasing the risk of stroke.
- Plaque rupture, leading to blood clots that can also cause strokes.

One treatment option is Carotid Angioplasty and Stenting, which is a less invasive procedure where a catheter is used to inflate a balloon in the artery, followed by placing a stent to keep it open.

Prostate Artery Embolisation (PAE) is a minimally invasive procedure that reduces the size of the enlarged prostate (or benign prostatic hyperplasia (BPH) by blocking the arteries that supply blood to the prostate gland. It is performed by inserting a catheter through the groin artery and injecting tiny particles into the arteries supplying the prostate. This causes the prostate to shrink, relieving urethral compression.

Find out if you have BPH, and consult our specialists if PAE could be the right solution.

Choosing the right tools for the right job. When it comes to treating aortic aneurysms, you deserve the best.

Our team of highly skilled and board-certified vascular surgeons has extensive years of experience and a proven track record in aortic aneurysm repair. Dr Benjamin Chua and Dr Chen Min Qi use the most advanced surgical techniques, including minimally invasive options, to promote quicker recovery and ensure best outcomes.

These techniques can be selected based on individual patient factors, including the size and location of the aneurysm, overall health, and specific anatomical considerations.

1. Endovascular Aneurysm Repair (EVAR) - A minimally invasive procedure where a stent graft is inserted through small incisions in the groin and guided to the site of the aneurysm. Benefits of this technique include shorter recovery time, reduced postoperative pain, and a lower risk of complications compared to open surgery.

2. Open Surgical Repair - A traditional approach involving a larger incision in the abdomen or chest to directly access the aorta and replace the aneurysmal section with a synthetic graft. This approach may be suitable for complex cases or when the aneurysm is large or in a difficult location.

3.Hybrid Procedure Repair - This combines open surgical techniques and endovascular methods, often used for patients with complex anatomy or multiple aneurysms. This is a tailored approach that maximizes the benefits of both techniques.

4. Fenestrated and Branched Stent Grafts - With this technique, specialized stent grafts designed with openings (fenestrations) or branches to preserve blood flow to vital vessels that may arise from the aorta. This is considered for challenging cases where traditional stent grafts are not suitable.

Venous conditions refer to disorders that affect the veins, which are responsible for returning blood to the heart.

Here are some common venous conditions:
1. Varicose Veins: Enlarged, twisted veins often visible under the skin, usually occurring in the legs.
2. Chronic Venous Insufficiency (CVI): A condition where the veins cannot pump enough blood back to the heart, leading to blood pooling in the legs.
3. Deep Vein Thrombosis (DVT): The formation of a blood clot in a deep vein, often in the legs.
4. Pulmonary Embolism (PE): A serious condition that occurs when a DVT clot travels to the lungs, blocking blood flow.
5. Venous Ulcers: Open sores that occur due to improper functioning of the venous system, typically seen in patients with CVI.
6. Superficial Thrombophlebitis: Inflammation and clotting in a superficial vein, often in the legs.
7. Venous Malformations: Congenital abnormalities in the veins that can lead to swelling or pain.

Venous conditions can significantly impact quality of life and may lead to serious complications if left untreated. Early diagnosis and appropriate management are crucial.
