A K-pop idol got second-degree burns from HIFU after 600 shots under sedation. HIFU burns are thermal injuries caused when ultrasound energy is delivered incorrectly during a High-Intensity Focused Ultrasound treatment. They are preventable. They come down to four factors:

1. Wrong cartridge depth for the facial area
2. Excessive energy settings
3. Poor practitioner technique
4. Gel application and device contact (the one most people overlook)

The gel is not just for comfort. Too little and the energy scatters, causing surface burns. Too much and the practitioner compensates with higher intensity. Poor handpiece contact concentrates heat on one spot.

Under sedation, there is no pain feedback. The doctor loses the one real-time signal that something is wrong.

This is why real-time imaging matters. Ultherapy uses live ultrasound imaging so the doctor can see skin layers and confirm depth before each shot.

”Is there anything I can do for acne scars without expensive lasers?“

Dr Jane Pang, Lead Aesthetic Doctor at EHA Clinic Singapore, gets asked this often. The answer depends on what you are actually dealing with.

Most people use ”acne scars“ to describe two different things. Flat discolouration after a breakout is a pigment issue — in many cases it fades on its own with consistent sun protection and appropriate topical support. True indented scars are structural, caused by collagen damage during healing, and need targeted treatment such as microneedling, subcision, or chemical peels.

Identifying the right type first is also what makes treatment cost-effective. The wrong approach, or the wrong order, is where budgets get stretched without results.

Clearing your acne was the hard part. But the marks and pits that remain are a separate problem, and they don’t respond to the same treatments.

What’s left behind falls into two categories. Flat discolouration, the dark, red, or purplish patches, is a pigment issue that can fade on its own over 3 to 12 months. The pits and uneven texture are structural, caused by collagen damage during healing. Those don’t resolve without targeted treatment.

The distinction matters because treating the wrong one wastes time. Dr Jane Pang, Lead Aesthetic Doctor at EHA Clinic Singapore, also flags a sequencing issue that often gets missed: if active acne is still present, that needs to be addressed first. Treating existing scars while new breakouts are forming means new scarring is being created faster than old scarring can be treated.

“Why isn’t this working?” is one of the most common questions Dr Jane Pang, our Lead Aesthetic Doctor, hears from patients who have already tried peels, lasers, and skincare routines.
The answer usually comes down to one thing: marks and scars are not the same condition. They look similar but they form at different skin layers, for different reasons, and they require entirely different treatments.
