Stroke

A stroke is a medical condition in which poor blood flow to the brain results in cell death. There are two main types of stroke: ischemic, due to lack of blood flow, and hemorrhagic, due to bleeding. Both result in parts of the brain not functioning properly. Signs and symptoms of a stroke may include an inability to move or feel on one side of the body, problems understanding or speaking, dizziness, or loss of vision to one side. Signs and symptoms often appear soon after the stroke has occurred. If symptoms last less than one or two hours it is known as a transient ischemic attack or mini-stroke. A hemorrhagic stroke may also be associated with a severe headache. The symptoms of a stroke can be permanent. Long-term complications may include pneumonia or loss of bladder control.

The main risk factor for stroke is high blood pressure. Other risk factors include to***co smoking, obesity, high blood cholesterol, diabetes mellitus, a previous TIA, and atrial fibrillation. An ischemic stroke is typically caused by blockage of a blood vessel, though there are also less common causes. A hemorrhagic stroke is caused by either bleeding directly into the brain or into the space between the brain's membranes. Bleeding may occur due to a ruptured brain aneurysm. Diagnosis is typically based on a physical exam and supported by medical imaging such as a CT scan or MRI scan. A CT scan can rule out bleeding, but may not necessarily rule out ischemia, which early on typically does not show up on a CT scan. Other tests such as an electrocardiogram (ECG) and blood tests are done to determine risk factors and rule out other possible causes. Low blood sugar may cause similar symptoms.
Prevention includes decreasing risk factors, as well as possibly aspirin, statins, surgery to open up the arteries to the brain in those with problematic narrowing, and warfarin in those with atrial fibrillation. A stroke or TIA often requires emergency care. An ischemic stroke, if detected within three to four and half hours, may be treatable with a medication that can break down the clot. Aspirin should be used. Some hemorrhagic strokes benefit from surgery. Treatment to try to recover lost function is called stroke rehabilitation and ideally takes place in a stroke unit; however, these are not available in much of the world.

TCM for stroke
From the perspective of Traditional Chinese Medicine, stroke is not just considered an illness of the brain. Stroke affects the meridians, other organs and blood vessels as well. It can be caused by internal and external influences that induce a buildup of qi and blood and create a high pressure state, which ultimately leads to a stroke. This condition is similar to the understanding in Western medicine which high blood pressure is a leading cause of stroke. Western medicine also suggests stroke can be caused by other factors such as smoking, heart disease, obesity, high blood cholesterol level and diabetes.

脑中风

脑中风是一组以脑部缺血及出血性损伤症状为主要临床表现的疾病,又称脑卒中或脑血管意外,具有极高的病死率和致残率,主要分为出血性脑中风(脑出血或蛛网膜下腔出血)和缺血性脑中风(脑梗塞、脑血栓形成)两大类,以脑梗塞最为常见。脑中风发病急,病死率高,是世界上最重要的致死性疾病之一。中风的死亡率也有随年龄增长而上升的趋势,由于一直缺乏有效的治疗措施,目前认为预防是最好的措施,因此,加强对全民普及脑中风的危险因素及先兆症状的教育,才会真正获得有效的防治效果。
脑中风的症状包括
1、头痛:无论是脑出血或脑梗死,剧烈头痛非常常见,
2、呕吐:一般是伴随头痛一起出现的,也非常常见,其特点是多为喷射状呕吐。
3、眩晕:眩晕还多伴有呕吐或耳鸣,也是脑中风的症状中比较常见的。
4、一侧肢体和面部的感觉异常,手足无力或不听使唤。
5、口角流涎(流口水):出现口角斜、流口水或食物从口角流出的现象。
6、突发的视感障碍:表现为复视,即看一个物体为两个物体。
7、突发的言语不清和吞咽呛咳症状:表现为病人说话不清,吐词困难,喝水或吞咽时呛咳。
8、意识障碍:变现为神志模糊不清、呼吸不应、打呼噜,严重的可出现深度昏迷。
在本地而言,由于中风发病急,病情严重,绝大多数中风患者都会选择入院接受西医治疗和急救。几乎没有中风患者会选择中医治疗。只有在之后的中风后遗症会接受中医的治疗。
脑中风后遗症

1.出血性脑中风
(1)肢体功能障碍主要表现偏瘫侧感觉和运动功能障碍
(2)认知和精神较大范围或多次复发的脑出血,可留有精神和认知障碍:如性情改
变、消极悲观、郁抑寡欢、精神萎靡、易激动等。
(3)言语功能障碍。
(4)吞咽功能障碍。
(5)其他症状头疼、眩晕、恶心、失眠、多梦、注意力不集中、耳鸣、眼花、多汗、
心悸、步伐不稳、记忆力减退、痴呆等。

2.缺血性脑中风
1)偏瘫是最常见的脑血栓后遗症。一侧肢体肌力减退、活动不利或完全不能活动。常伴有同侧肢体的感觉障碍如冷热不知、疼痛不觉等。
(2)失语运动性失语表现为病人能听懂别人的话语,但不能表达自己的意思。感觉性
失语则无语言表达障碍,听不懂别人的话,也听不懂自己所说的话,表现为答非
所问。命名性失语则表现为看到一件物品,能说出它的用途,但却叫不出称。
(3)较大范围或多次复发也是脑血栓后遗症的症状可留有精神和智力障碍:人格改
变、消极悲观、郁抑寡欢、精神萎靡、易激动等。
(4)其他症状头疼、眩晕、恶心、失眠、多梦、注意力不集中、耳鸣、眼花、多汗、
心悸、步伐不稳、颈项酸痛疲乏、无力、食欲不振、记忆力减退、不能耐受噪声
等。

中医对中风后遗症的概念
《金匮要略》将喎僻不遂、不识人、舌强难言统一为中风,在《中风历节病脉证并治第五》第 2 条指出,“邪在于络,肌肤不仁;邪在于经,即重不胜;邪入于腑,即不识人;邪入于脏,舌即难言,口吐涎”明确提出中风的病名。
脑中风后遗症是指中风发病后遗留不同程度的半身不遂、肢体麻木、言语蹇涩、口舌㖞斜、痴呆、吞咽困难、呛食、呛水等症状,又称为“脑中风后遗症“。
中医通过辨证论治以针灸和中药治疗是有一定的特殊疗效。我个人认为在中风发作后,经过急救处理,病情稳定后的1~3个月里的恢复期中配合中医针灸与中药治理是黄金期。

认识中风先兆
中风先兆,即指中风发病前表现出的各种症候群。这些症状预示中风可能发生或即将来临,特点介乎于中风显症与隐症之间,主要表现有:头痛、头晕、恶心,甚至出现一过性失语、失明,可伴有健忘、记忆力减退、注意力不集中,以近期遗忘、尤其人的姓名遗忘为甚,但理解力及远期记忆良好。感觉异常:以肢麻、指麻常见。动作异常:以头摇、肌肉眶动、口角撮动、眼皮跳动、行走易跌绊。上述症状不一定每个患者均有,约有30%中风患者在发病前几乎没有任何先兆,这一方面与病变性质、病变程度有关,另一方面与患者的感觉及敏感性等个体因素相关。
中医学考虑为:中风先兆病机属于虚实夹杂。肝肾阴虚,气血衰少是本虚;痰痹阻络、气血失运为标实,两者互为因果,最终导致脑髓空虚、脑脉气血不运,出现上述临床症状。在对中风先兆认识的基础上,我们如果能够对出现中风先兆的患者给予适时的中医针灸干预,这将会极大降低中风的发病率,从而真正体现中医未病先防的治未病思想。

老人频咬舌头竟是“中风先兆”
  吃饭频咬舌头,夹菜总是夹空,六旬老人入院就诊,发现中风先兆。62岁的刘爹爹家住百步亭,平日很喜欢热闹,最近却一反常态变得沉默寡言。上周,一家人围在一起吃火锅,刘爹爹筷子频频夹空,还时不时咬到舌头,这一反常情况引起家人警觉。饭后,刘爹爹在家属陪伴下到湖北省中医院脑病科就医,检查证实为短暂性脑缺血。
中医如何治疗中风先兆?
中风先兆已经出现,在此阶段,机体功能低下,经络暂时痹阻不通,机体经气不能被有效的激发,可针对不同证候,施以相应针灸疗法,截病于早期,避免中风的发生。①肝阳化风:症状表现为:眩晕、头重脚轻、肢麻,甚则卒然言语蹇涩但瞬时即过,腰膝酸软,舌质红,薄黄苔或少苔,脉弦硬而长或寸盛尺虚。
②痰瘀互结:症状表现为:头晕目眩,头重如蒙,胶麻,或卒然半身不遂,过时恢复如常,舌质暗淡,苔白腻或黄厚腻,脉弦硬而滑或滑数
。③气虚血瘀:症状见半身麻木,或一侧肢体软弱,偶有肢体颤动,夜卧口角流涎,夜尿频数,舌质淡紫,脉弦虚而大。
④髓空窍瘀:症见头晕耳鸣,健忘,肢体麻木,腰膝酸软,神情呆滞,夜寐多梦,舌淡微紫,脉象细涩。

中医论中风概念及病因
病因病机中风的发病突然,是其病理是逐渐形成的。与肝、肾、心、脾的关系最为密切,其病因与虚、风、痰、火四者密切相关,发病机理罗为复杂。
中风:是以猝然昏仆,不省人事,伴有口眼喎斜,语言不利,半身不遂的一种疾病。因其发病骤然,变化迅速,似有“风性善行而数,故称为“中风”,又名“卒中”,“类中”等。西医的脑出血、脑血栓形成、脑栓塞、脑血管痉挛、蛛网膜下腔出血等多种脑血管疾病,均属本证论治范畴。
(1)情志所伤或所老肾衰,致阴阳失调,发为本病。或因暴努伤肝,使肝阳暴动,引起心火,风火相扇,气热郁逆,气血并走于上,心神昏冒而发病。
(2)过食肥甘或劳倦伤脾,或肝阳素旺,横逆犯脾,脾失健运,痰湿内生,或肝火内灼,灼烁液成痰,痰郁化火,蒙敝清窍,流窜经络而卒然发病。
(3)脉络空虚,风邪内侵,中于经络气血痹阻,肌肉筋脉失于濡养,;或形盛气衰,痰湿素盛,外风引动痰湿,闭阻经络而致喎噼不遂。
如何预防中风
治病不如防病,中风作为一种致残致死率极高的病,给病人及家属带来的痛苦是不可想象的,那么,我们该怎样预防呢?

1.控制高血压
高血压是导致中风的根源之一。高血压病人要遵医嘱,按时服用降压药物,有条件者最好每日测1次血压,特别是在调整降压药物阶段,以保持血压稳定。要保持情绪平稳,少做或不做易引起情绪激动的事,如打牌、搓麻将、看体育比赛转播等;饮食须清淡有节制,戒烟酒,保持大便通畅;适量活动,如散步、打太极拳等。
2.防治动脉粥样硬化
要防治动脉粥样硬化,关键在于防治高脂血症和肥胖。建立健康的饮食习惯,多吃新鲜蔬菜和水果,少吃脂肪高的食物如肥肉和动物内脏等;适量运动增加热量消耗;服用降血脂药物。
3.注意中风的先兆征象
一部分病人在中风发作前常有血压升高、波动、头痛头晕、手脚麻木无力等先兆,发现后要尽早采取措施加以控制。
4.注意短暂性脑缺血发作
当病人有短暂性脑缺血发作先兆时,应让其安静休息,并积极治疗,防止其发展为脑血栓。
5.注意气象因素的影响
季节与气候变化会使高血压病人情绪不稳,血压波动,诱发中风,在这种时候更要防止中风的发生。
6.多吃果蔬
蔬菜和水果中含有大量维生素C。据研究,血液中维生素C浓度的高低与脑中风密切相关,浓度越高,脑中风的发病危险就越低。此外,许多果蔬中含有寡糖(又称低聚糖),有减少血流凝集的作用,也可以防止中风。

TCM Treatment of Urticaria (with a case study)

Urticaria (Hives) is defined as a kind of skin rash notable for pale red, itchy bumps caused by allergic reactions to internal and external agents. The word ‘urticaria’ is derived from the Latin word urtica, which means ‘nettle’, which is a tooth-leaved plant covered with hairs that secrete a stinging fluid that immediately affects the skin.

According to TCM, the following factors may be involved in urticaria:
- sensitive to substances, food, drugs, biological products, infections, intestinal parasites, etc.
- emotional factors
- external weather changes, such as wind, cold, etc.

Urticaria is a disease at the body surface, but it is always a manifestation of the internal organs. In TCM, it is said that “Viscera inside the body must manifest themselves externally”. “有诸内,必形诸外” “Viscera” refers to the internal organs inside a human body. Disharmony within the internal organs will be reflected externally.

In TCM, Urticaria is also known as Feng Mo or Feng Tuan (wind rash). It occurs in 2 forms: acute and chronic.

Acute urticaria is generally attributed to an external invasion of Wind-Heat or Wind-Cold induced by Lung and/or Spleen Qi imbalance.
When the Lung Qi is deficient, the invasion occurs through the skin or respiratory tract after exposure to pollens or irritants. When the Spleen Qi is affected, there is an accumulation of Damp-Heat, which then manifests in the skin after exposure to triggers such as shellfish. Hence, diet is important in the management of urticaria as well.

Chronic urticaria is usually attributed to an internal stirring of Wind and Heat, in association with Blood deficiency.

TCM treatments aim to restore the internal balance so that it is internally balanced with external environment. A combination of both Acupuncture and Chinese herbal medicine is usually given to help manage the symptoms and to prevent acute onsets in the case of chronic urticaria.

Here is an example of a case study:
Case Study: Female, age 40+ Date of visit: 25 Feb 2019

She first started having hives outbreaks since June 2018. She was in the process of moving to a new home during Aug 2018. She was prescribed antihistamines by a doctor and was taking them for 2 months. However, whenever she took the western medicine, she would feel drowsy and the condition persisted, hence, she stopped taking the western medicine. She was referred by her friend and came to visit Physician Poh on 25 Feb 2019.

Clinical Presentations: Skin Scratch test (positive), recurrent outbreaks of hives (itchy red bumps) in the lower back and thighs regions, prone to feeling cold easily, dry lips, dark coloration of tongue and thick white coating, stringy pulse

Triggers: After eating sweet or savory foods or after bathing at night 8-9pm

Analysis: Lack of rest and stress during the shifting to a new home could have caused her immune system to be weakened. A possible trigger factor could be a change in environment when moving to a new temporary place (her sister’s place) to stay while the new home was undergoing renovations. She has a weak digestive system and hence the thick coating on the tongue, indicating dampness and toxins accumulated in the body were not cleared out quickly enough. There was underlying stress as well which caused Qi (vital energy) and blood stagnation.

Treatment: Acupuncture was done and a week of Chinese herbal medicine was prescribed.

Points such as Xue Hai, Zu San Li, He Gu, Tai Chong were chosen to help promote Qi and blood circulation, disperse wind and boost the digestive system.

Chinese Herbal medicine consists of modified *Xiaofeng powder with the aim to dispel wind, eliminate dampness, clear heat and promote Qi and blood circulation.

Second Visit: 6 Mar 2019

After a week of Chinese herbal medicine, she complained of having ulcers. She still had a few episodes of hives (still itchy) on her thigh regions but the frequency had reduced. Her tongue was darkish in color and the thick white coating in the tongue has reduced a little but with a tint of yellow coloring (damp-heat).

Treatment: Acupuncture was done and a week of Chinese herbal medicine was prescribed.

Modification of Chinese Herbal medicine: modified Xiaofeng powder and addition of Yin Qiao San to further disperse wind-heat and relieves toxicity.

A week later, the patient feedback that she was feeling fine and no more onsets of hives. Subsequently, in May and June, she continued with acupuncture treatment and taking 3 weeks of Chinese herbal medicine each time. The objective of once-a-month TCM treatment sessions was to help strengthen her immune system, thus preventing future episodes of hives and for regular maintenance.

*Points to note: The medicine formulas listed in the case study were prescribed according to the patient’s body constitution and the patterns of disharmony which she presented (TCM Syndrome Differentiation). Do not purchase such formulas on your own. It is advisable to seek consultation from a registered TCM practitioner to manage your condition holistically.

湿疹Eczema 是一种慢性(长期)皮肤病,其表现形式多种多样。病情可能非常轻微,也可能极为严重。湿疹是儿童常见的一种皮肤病。有时儿童病患,随着年龄增长,其症状也会有所缓解,甚至完全消退。部分患儿的湿疹病情则会伴其终身。。
湿疹患者会出现皮肤干痒,并可能起皮疹。病情可能时好时坏。病情加重时,我们称之为“炎症发作”。空气干燥,炎症最常发作,但在不同的气候环境也可能出现病情加重的情况。
目前尚不清楚湿疹的病因,遗传因素和内外环境因素都有可能是诱因之一。湿疹患儿也可能患有哮喘或过敏症(如花粉症)。

湿疹的征兆和症状
根据病情的严重程度,湿疹的征兆和症状也可能有所差异。
湿疹的征兆和症状包括:
皮肤瘙痒 ,皮肤干燥, 皮肤发红, 湿疹斑块(屑状皮疹), 睡眠不足容易促症状加剧。皮肤发痒
抓痕后,肤色发生变化,情况会更严重时,患处可能会流出透明液体,或结痂、脱屑

湿疹是现在很多人都会患上的一种常见的皮肤病,其实导致人们患上湿疹的原因有很多,而且也非常复杂。如果人们想要治疗湿疹,那么就必须了解导致自己患上湿疹的原因是什么,这样才能够从源头上着手,真正的治好湿疹,不再让它复发。

第一,生活习惯
大部分的人会患上湿疹都是因为自己的生活习惯不好,如果一个人想避免自己患上湿疹,就要少接触含有化学成分的用品,并且要使用自己身体能够接受的水温来洗澡、洗头和洗脸。

第二,过敏
除了不良的生活习惯之外,有很多人是因为过敏了之后才会出现湿疹的情况。当然,导致人们出现过敏的情况也是有很多不同的原因。有可能是吸入了空气中的粉尘、植物的花粉或者是棉花絮等等,也有可能是吃了海鲜、花生、牛肉和羊肉这样的食物,甚至还可能是接触到了化妆品和油漆这样的物品。所以人们在生活中最好能去医院进行一个过敏源的检查,避免接触会导致自己过敏的东西,这样才能够很好的预防湿疹的出现。

第三,皮肤过于干燥
如果一个人经常在外面晒太阳、皮肤长期处于非常干燥又或者湿热的情况,那么这个人很有可能就会出现湿疹的情况。在这样的情况下,皮肤就会变得非常痒,如果人们用手去抓,就会导致情况变得越来越严重。

第四,其他因素

中医对湿疹的发展与认识

《内经》最早认为湿疹的主要原因是心火太过.
张仲景认为”浸淫疮”(湿疹)是由于湿热所致,他也认为湿疹不仅与心火太过有关,而且还与外因湿热有关.这就对湿疹病因病机的认识更进了一步.

隋代不仅继承了前人的经验,而且认识到手足部慢性湿疹的病因病机是肌腠虚,风湿与血气相搏,郁于肌肤而发.突出了风邪在本病发病中的作用,并且认识到耳部湿疹的发作具有周期性.到了明代中医认识到足部的湿疹是风湿血热交感而成.《外科正宗》中记载:“儿在胎中,母食五辛,父餐炙煿,遗热于儿,生后头面遍身发为奶癣,流脂成片,睡卧不安,搔痒不绝.”可见明代已经认识到婴儿湿疹与父母的饮食有关.清代《医宗金鉴.外科心法要诀》云:“浸淫疮,初生如疥,搔痒无时,蔓延不止,抓津黄水,浸淫成片,由心火脾湿受风而成.”指出了湿疹产生的内因为心火脾湿,外因为风邪.医家基本认为手足部湿疹是由于风邪与湿热相搏于肌肤而成.

Eczema

Eczema is a chronic skin disease that manifests itself in a variety of forms. The symptoms may vary from being mild to being extremely severe.
Eczema is common skin condition for children. In some cases as the child ages, the symptoms may be alleviated or even completely resolved. However others may have the condition throughout their later lives.

Eczema patients may experience constant or intermittent dry skin, itching or rashes. When the condition worsens, we call it "inflammation episodes." Inflammation is more common in dry weather, but may also occur in different climates.
The cause of eczema is currently still unclear. Genetics, internal pathological factors and environmental factors may be one of the triggers. Children with eczema may also have asthma or allergies (such as hay fever).

Signs and symptoms of eczema
The signs and symptoms of eczema may vary depending on the severity of the condition.
The signs and symptoms of eczema include:
Itchy skin, dry skin, rashes, thickened skin, redness and swelling. Symptoms may seem worse if patient has lack of sleep.

Due to the itchiness, the patient may tend to scratch the affected areas which will ultimately cause the skin tone to change. As the condition worsens, the affected area may experience fluid drainage and blistered skin.

Eczema is now a common skin disease that affects many and its causes are very complicated. In order to treat eczema, one must first understand the cause, so prevent it from recurring.

First, living habits
Most people suffer from eczema due to poor living habits.
Minimize direct contact with daily products that has high chemical content. Bathe regularly at a water temperature that is suitable for you.

Second, allergies
Allergies are also a common contributing factor.
There are many different types of allergies ranging from dust, pollen, cosmetic products and food allergies such as seafood, peanuts, etc.
It is strongly encouraged for patients to have a skin prick test to understand their allergic reactions.

Third, the skin is too dry
Excessive exposure to the sun as well as hot and humid weather can also cause the aggravation of eczema.

Fourth, Others
In some instances, Eczema may be triggered by extreme stress, nervousness, insomnia, hormonal changes and body metabolism disorder. Eczema caused by these factors is usually temporary, and can be cured quickly if it is treated.
