เมื่อต้นสัปดาห์ที่ผ่านมาหมอได้มีโอกาสช่วยสอนผ่าตัดกระจกตา โดยเฉพาะการผ่าตัดใส่วงแหวนเฟอร์ราร่าในกระจกตา(ICRS) และการผ่าตัดใส่ชิ้นกระจกตาเสริมในเนื้อกระจกตา(CAIRS) ซึ่งทั้งสองวิธีเป็นวิธีในการรักษาโรคกระจกตาโป่ง ในงานประชุมประจำปีครั้งที่ 52 ของราชวิทยาลัยจักษุแห่งประเทศไทย

Early this week, I had an opportunity to be a surgical instructor for cornea surgery especially Intrastromal corneal ring segment implantation (Ferrara ring implantation) and Cornea allogenic intrastromal ring segment implantation (CAIRS) at the 52nd meeting of Royal College of Ophthalmology meeting of Thailand. Both surgeries are the current treatment for keratoconus.

เป็นเกียรติอย่างยิ่งค่ะที่ได้รับเชิญเป็นหนึ่งในผู้ร่วมอภิปรายในหัวข้อการเปลี่ยนกระจกตาเฉพาะชั้นใน (Endokeratoplasty: EK ซึ่งรวมทั้งวิธี DSAEK และ DMEK) ร่วมกับแพทย์ผ่าตัดเปลี่ยนกระจกตาที่มืชื่อเสียงระดับโลกในงานประชุมปีนี้ของ The international Society of cornea, stem cells and ocular surface (SICSSO) congress ที่จัดที่เมืองกรอสเสตโต้ ประเทศอิตาลีระหว่างวันที่ 27-29 มิถุนายน 2567

การผ่าตัดเปลี่ยนกระจกตาเฉพาะชั้นในที่หมอเริ่มทำเป็นตาแรกเริ่มต้นเมื่อ 16 ปีที่แล้ว ที่โรงพยาบาลจักษุรัตนินค่ะ เป็นการผ่าตัดด้วยวิธี DSAEK โดยหมอเป็นผู้แยกชั้นกระจกตาบริจาคก่อนผ่าตัดเอง กระจกตาที่ใช้เป็นกระจกตาบริจาคที่ได้รับจากสภากาชาดไทย เราใช้ส่วนหลังสำหรับผู้ป่วยDSAEKรายแรกค่ะ ส่วนกระจกตาบริจาคส่วนหน้าถูกนำไปใช้กับผู้ป่วยเด็กของหมออีกรายที่ได้รับการผ่าตัดเปลี่ยนกระจกตาเฉพาะส่วนหน้าค่ะ กระจกตาบริจาคหนึ่งดวงจึงได้นำมาใช้กับผู้ป่วยสองราย การผ่าตัดเป็นไปได้ด้วยดี กระจกตาของผู้ป่วยใสจนวันสุดท้ายของชีวิตค่ะ กระเป๋าสตางค์ใบนี้เป็นของขวัญปีใหม่ที่หมอได้รับจากผู้ป่วยท่านนี้ค่ะ ในกระเป๋ามีกระดาษสองใบเล็กๆ เขียนไว้ว่า ขอให้สุขภาพแข็งแรง ร่ำรวยด้วยเงินทองนะคะ รักคุณหมอจากป้า และอีกแผ่นเขียนไว้ว่า ตามความเชื่อของโบราณ ให้กระเป๋าใส่ต้องมีเงินก้นถุง รัก จากป้านะคะ ผู้ป่วยยังให้เบอร์โทรศัพท์มือถือทิ้งไว้เพื่อให้ผู้ป่วยที่หมอจะทำผ่าตัดเป้นรายที่สองสามารถสอบถามเกี่ยวกับการผ่าตัดโดยตรงกับท่านได้ด้วย

ในโอกาสนี้ หมอขอขอบคุณผู้ป่วย DSAEK และ DMEK ที่ทำให้หมอได้มีวันนี้นะคะ

It has been such an honor to be among the world authorities in cornea transplant as an invited panelist in the session of the Endothelial Keratoplasty (EK: include DSAEK and DMEK) at this year The international Society of cornea, stem cells and ocular surface (SICSSO) congress in Grosseto, Italy.

My selective posterior lamellar keratoplasty ( Endokeratoplasty or EK) was first performed 16 years ago at Rutnin Eye Hospital, I used my own surgeon-prepared graft for my first DSAEK surgery. The corneal tissue was from Thai red Cross. I used one donor tissue for two recipients, one anterior lamellar cornea transplant patient and this first posterior lamellar cornea transplant (DSAEK surgery) patient. The surgery went well. The graft was still clear until the day the patient passed away. This purse, as a new year gift from her, reminded me about her. There were two little notes and 999 bahts in the purse. In the notes, she wished me healthy and wealthy and explained to me that according to the ancient beliefs, there should be some money in the purse as a starter amount for good luck. She was also willing to gave me her cellphone number for my second EK patient to call her and ask her everything about the surgery.

I would like to take this opportunity to thank all of my DSAEK and DMEK patients. I would have not been this far without my patients.

ผู้ป่วยพม่าเคยรับการผ่าตัดต้อกระจกมาก่อนมีอาการตามัวเคืองตามาหลายปี ตรวจพบมีแผลเป็นนูนที่ผิวกระจกตา กระจกตาบวม เลนส์ตาอยู่หน้าม่านตา ม่านตาโตและเนื้อม่านตาส่วนบนขาดหายไป

ห้าเดือนก่อนผู้ป่วยได้รับการผ่าตัดเอาแผลเป็นนูนออก เปลี่ยนเลนส์ตาไปอยู่หลังม่านตา และเย็บรูม่านตาให้เล็กลง หลังผ่าตัดกระจกตายังบวม รูม่านตายังใหญ่กว่าปกติ

หนึ่งเดือนก่อนผู้ป่วยมารับการผ่าตัดเปลี่ยนกระจกตา เนื่องจากรูม่านตาโตกว่าปกติมากจึงพิจารณาผ่าตัดเปลี่ยนกระจกตาด้วยวิธี DSAEKซึ่งใช้กระจกตาที่ปลูกถ่ายหนาประมาณ 100 ไมครอน เพราะหากทำผ่าตัดด้วยวิธี DMEK กระจกตาที่ปลูกถ่ายด้วยวิธี DMEKซึ่งมีขนาดบางเพียง 20 ไมครอนอาจจะตกลงไปส่วนหลังของม่านตาผ่านรูม่านตาที่โตกว่าปกติได้ ผู้ป่วยรายนี้หลังผ่าตัดวันแรกฟองอากาศที่ใส่ไว้ไปอยู่เลนส์ตาทั้งหมดผ่านรูม่านตาที่เป็นช่องต่อเนื่องระหว่างช่องส่วนหน้าและส่วนหลังของตาทำให้ผู้ป่วยเริ่มมองเห็นได้ตั้งแต่วันแรก การฟื้นตัวหลังผ่าตัด DSAEKมักใช้เวลาประมาณสามเดือน

This Myanmar patient had complicated cataract surgery many years ago. He had eye irritation and poor vision for many years. Eye examination showed dense corneal scar on the corneal surface, cornea swelling, intraocular lens implant located in front of the iris, fixed and dilated pupil and iris tissue loss.

Five months ago, he had undergone corneal scar removal, intraocular lens exchange and pupilloplasty. After this surgery, the pupil was still big. There was an iris defect on the superior part and cornea swelling.

One month ago, the patient came back for selective posterior lamellar cornea transplant. In this case, DSAEK surgery was recommended. If DMEK surgery is performed, the DMEK graft which has the thickness of only 20 microns, may dislocate into the posterior part of the eye through the big pupil or the iris defect. DSAEK graft of 100 microns thickness is more controllable during the surgery compare to the DMEK graft. In this case, the air bubble was gone from the anterior chamber from the first day after the surgery so the patient could see from first day after the surgery. Visual recovery after DSAEK surgery usually takes three months.

ผู้ป่วยมีกระจกตาบวมเล็กน้อยจากเลเซอร์ม่านตาที่เคยทำเมื่อสิบกว่าปีก่อน และมีต้อกระจก ในกรณีนี้หากทำการผ่าตัดต้อกระจกเพียงอย่างเดียว หลังผ่าตัดกระจกตาจะบวมมากขึ้น ทำให้ตามัวและเจ็บเคืองตาได้

ผู้ป่วยรายนี้จึงจำเป็นต้องได้รับการผ่าตัดต้อกระจกและผ่าตัดเปลี่ยนกระจกตาเฉพาะชั้นในด้วยวิธี DMEK หลังผ่าตัดหนึ่งชั่วโมงแรก มีฟองอากาศในตา 80% ดังรูป

This patient had cornea decompensation (swelling) from post laser iridotomy she had undergone more than ten years ago. She also had cataract. In this case, if she has only cataract surgery, the cornea will be more decompensated. Her vision will be foggy and she will have eye irritation after the surgery.

This patient needed combined cataract surgery and selective posterior lamellar cornea transplant (DMEK) surgery. One hour after the surgery, the air occupied 80% of the anterior chamber as seen in the pictures.

อุบัติเหตุที่เกิดขึ้นได้ในผู้ที่เคยรับการผ่าตัดเลสิคมาก่อน

ผู้ป่วยรายนี้เคยผ่าตัดแก้ไขค่าสายตาด้วยวิธีเลสิคมาก่อนเมื่อสิบกว่าปีก่อน หนึ่งสัปดาห์ก่อนไปลองเสื้อแล้วป้ายเสื้อมาโดนบริเวณตาซ้าย หลังจากนั้นรู้สึกเคืองตามาตลอดและตามัวลงจึงมาตรวจตา ตรวจพบว่าขอบของฝากระจกตาเปิดและพับเข้าด้านใน (ลูกศรสีแดง) และมีเซลล์ผิวกระจกตาเป็นแผ่นสีขาวงอกอยู่ใต้ฝากระจกตา (ลูกศรสีเขียว)

ผู้ป่วยได้รับการผ่าตัดคลี่กระจกตาให้เข้าที่ ขูดเอาเซลล์ผิวของกระจกตาที่อยู่ใต้ฝากระจกตาออก และเย็บฝากระจกตาเพื่อป้องกันไม่ให้ผิวเซลล์กระจกตางอกเข้าไปใต้ฝากระจกตาอีก ภาพหลังผ่าตัดหนึ่งเดือนฝากระจกตาเข้าที่ดี ผู้ป่วยกลับมามองเห็นปกติ

ผู้ที่ได้รับการผ่าตัดเลสิคหากได้รับอุบัติเหตุเกี่ยวบริเวณขอบฝากระจกตาพอดีจะทำให้เกิดเหตุการณ์เช่นนี้ได้ ผู้ที่ได้รับการผ่าตัดแก้ไขค่าสายตาด้วยวิธีอื่นได้แก่ วิธีเลเซอร์ที่ผิวกระจกตา(PRK) และรีเล็กซ์สไมล์ (Relex smile) ซึ่งไม่มีฝากระจกตาจะไม่มีอุบัติเหตุเช่นนี้เกิดขึ้น

LASIK flap dislocation

This patient had undergone LASIK surgery of both eyes more than ten years ago. She had left eye trauma from price tag injury a week before she came to see me. She had eye irritation and blurred vision. Eye examination showed the edge of the LASIK flap folded underneath the flap (red arrows). The green arrow showed surface epithelial cells that grew under the flap (as called epithelial ingrowth).

This patient had undergone LASIK flap reposition, epithelial ingrowth removal and cornea flap suturing to prevent recurrent epithelial ingrowth. The lower pictures showed the eye one month after the surgery. The patient gained back her normal vision.

Post LASIK patients could have LASIK flap dislocation like in this case. In post Photorefractive keratectomy (PRK) and post Relex Smile patients will not have this injury because there is no flap creation from the surgery.

ผู้ป่วยมีประวัติเคยได้รับการผ่าตัดตามาหลายครั้ง ได้สูญเสียการมองเห็นแล้ว หกปีก่อนมาตรวจครั้งแรกด้วยเรื่องปวดตาตาแดง ตรวจพบว่ามีการติดเชื้อของกระจกตาและกระจกตาบวม (ลูกศรสีแดง) ได้รับการรักษาติดเชื้อจนดีขึ้นกลายเป็นแผลเป็นแต่ยังมีอาการเคืองและเจ็บตาจากกระจกตาบวม จึงได้รับการผ่าตัดคลุมกระจกตาโดยใช้เยื่อบุตาของผู้ป่วยเองเพื่อลดอาการเจ็บเคืองตา

ในกรณีที่ผู้ป่วยมีกระจกตาบวมและพยากรณ์โรคได้ว่าจะไม่สามารถมองเห็นได้ดีขึ้นด้วยการผ่าตัดใดๆแล้ว แพทย์จะไม่แนะนำให้ผ่าตัดเปลี่ยนกระจกตา แต่จะแนะนำให้ผ่าตัดคลุมผิวกระจกตาเพื่อลดอาการเจ็บเคืองตา โดยสามารถทำได้สองวิธีคือด้วยการใช้เยื่อบุตาของผู้ป่วยเองเช่นในผู้ป่วยรายนี้ หรือใช้เยื่อรกในกรณีที่ไม่สามารถใช้เยื่อบุตาของผู้ป่วยได้เพราะเยื่อบุตามีแผลเป็นจากการผ่าตัด ภาพแสดงตาหกปีหลังผ่าตัด ไม่มีอาการเจ็บเคืองตาแล้ว

This patient had undergone multiples ocular surgeries and lost his vision. On his first visit six years ago, he had severe eye pain and redness. Eye examination showed corneal infection and swelling (red arrow). After the treatment, the infection was controlled. The corneal scar remained and cornea swelling (green arrow) caused pain and eye irritation. He had undergone conjunctival flap surgery for the treatment of his eye pain.

In the case of cornea swelling in patient with poor prognosis for vision, cornea transplant is not recommended. The aim of the treatment is to get rid of pain only. There are two options. The first one is conjunctival flap surgery, like in this case. Another option is amniotic membrane transplantation (AMT), which is done if conjunctival flap cannot be performed due to conjunctival scarring. Six years after the surgery, the patient is still doing well.

ผู้ป่วยรู้สึกเคืองตามาหลายวัน อาการไม่ดีขึ้นจึงเข้ามาตรวจ ตรวจพบมีสิ่งแปลกปลอม(ลูกศรสีแดง)ติดอยู่บนกระจกตา พลิกเปลือกตาดูไม่มีสิ่งแปลกปลอมติดใต้เปลือกตา ผู้ป่วยจึงได้รับการเอาสิ่งแปลกปลอมออกจากกระจกตา ในกรณีนี้เนื่องจากเป็นมาหลายวันจึงมีเส้นเลือดจากตาขาวเข้ามาในกระจกตาและมีแผลเป็นจางๆรอบบริเวณที่สิ่งแปลกปลอมติดอยู่ (ลูกศรสีเขียว)

This patient had eye irritation for many days. A foreign body (red arrow) was seen only on the corneal surface. It was then removed. In this case, the foreign body was left on the eye for many days. This induced blood vessels into the cornea and caused corneal scar formation (green arrow).
