July is Sarcoma Awareness Month...
I was deeply horrified to have seen in the same consultation, yesterday, two young teenagers (a girl and a boy) coming together from a neighboring country, each presenting metastatic osteosarcoma with vascular-nervous invasion, and leaving no choice except amputation.
This Sarcoma Awareness Month is an ideal opportunity to raise awareness among the public, primary care physicians and public authorities to no longer see patients with sarcoma for whom the oncology surgeon cannot provide the best possible outcome.

Megaprosthesis constitutes an excellent method of reconstruction in the surgery of bone sarcomas. However, it has specific complications such as aseptic loosening, a nightmare for the patient and the medical staff.
Here is a painful aseptic loosening of a distal femur prosthesis that appeared 11 years later after resection of osteosarcoma.
What was done?
Removal of the prosthesis, removal of the cement, and repositioning of the same prosthesis. Before, we made a cortical reconstruction of the distal diaphyseal shaft by allogeneic cancellous bone rings. #

Since I started seeing the mechanical complications of tumor reconstruction prostheses, I stopped liking them.
But they say only fools don't change their minds...
I see today this lady, currently 37 years old, whom I had operated on for osteosarcoma of the right proximal tibia. The reconstruction was made by a megaprosthesis.
I had changed the polyethylene bearings and the hinge only once.
She got married last year and she takes good care of her husband. She is happy.
Currently (22 years follow-up), knee function is good and the patient is satisfied despite a little prosthetic laxity.

Dans la LCH diagnostiquée tardivement, le traitement nécessite une réduction à ciel ouvert, une libération des tissus mous et une capsulorraphie. La réduction par voie antérieure est la plus utilisée.
En opérant beaucoup d'enfants, j'ai constaté que dans certains cas, la réduction de la tête fémorale et la capsulorraphie peuvent être réalisées sans ouvrir la capsule, c'est ce que j'ai appelé : la capsulorraphie extra-articulaire.
Cette variante de capsulorraphie pourrait éviter les complications de la réduction à ciel ouvert classique.
In late-diagnosed DDH, soft tissue contracture, inverted limbus, and capsular constriction hinder reduction. Treatment, therefore, requires an open reduction, soft tissue release, and capsulorrhaphy. Anterior open reduction is the most commonly used.
By treating many patients, I have found that in some cases, reduction of the femoral head and capsulorrhaphy can be performed without opening the capsule, this is what I called: Extra-Articular Capsulorrhaphy.
This variant of capsulorrhaphy could avoid complications of classic open reduction.
