Son 4 yıldır sol boyunda giderek büyüyen şişlik şikayetiyle başvuran 34 yaşındaki kadın hastamızın tetkiklerinde; sol şah damarı üstüne oturan yaklaşık 5x5 cm boyutlarında kitle saptandı. Kitleyi etrafındaki önemli damar ve sinir yapılarını koruyarak çıkardık.

HAFTANIN VAKASI:
Küçüklükten beri sık kulak enfeksiyonu geçirdiği için sol kulak zarında ileri derecede kalınlaşma ve işitme testinde "hafif derecede işitme kaybı" saptadığımız 49 yaşındaki kadın hastamızın kulak zarı, kulak tomografisi ve işitme testi görüntüleri.

Kulağına aldığı darbe sonrası 5 gündür sol kulağında uğultu, dolgunluk ve işitme kaybı şikayeti ile başvuran 32 yaşındaki kadın hastamızın muayenesinde kulak zarında yeni (taze) gelişmiş bir yırtık gördük ve lokal anestezi altında taze kulak zarı yırtığını yama (sigara kağıdı) ile tamir ettik.
Bu işlemi endoskop eşliğinde herhangi bir kesi yapmadan gerçekleştirdik ve hastamızı aynı gün taburcu ettik. Hastamızın DUYMASI ANINDA NORMALE DÖNDÜ ve KULAĞINDAKİ UĞULTU GEÇTİ👍
Fotoğraflarda; işlem öncesi ve 2, 5 ay sonrası;
sol kulak zarının endoskopik görünümü ve işitme testleri görülmektedir.

ℹ️ TAZE KULAK ZARI YIRTIĞINI YAMA İLE TAMİR NE DEMEKTİR? NE ZAMAN YAPILIR?

✅ Kulağa gelen ani fiziksel travmalar (tokat gelmesi, vurma, çarpma, denize-havuza atlama/düşme, kar topu gelmesi vb.) ve basınç etkisi (şiddetli hapşırma, uçakta iniş/kalkış sırasında) kulak zarında yırtılma tarzında deliğe yol açar.

✅ Kulak zarında oluşan bu taze yırtık, hasta İLK BİRKAÇ GÜN içinde başvurursa yama ile tamir edilebilir. Çünkü taze yırtık etrafındaki kan pıhtıları, zar dokusu üzerine konan yamanın altından ilerleyerek deliği kapatır. Başvuruda geç kalınırsa, genel anestezi altında kulak zarı deliği tamiri (timpanoplasti) ameliyatı yapılmak durumunda kalınır.

4 yıldır ilaç tedavileri ile küçülmeyen ve her iki boynunda 3x4 cm. lik şişlik şikayeti ile başvuran 7 yaşındaki erkek hastamızın boynundaki kitleyi çıkardık.
📍Patoloji raporu: Reaktif foliküler hiperplazi
(iyi huylu lenf bezi büyümesi)
Hastamızı takibe devam edeceğiz.

✅ Mutlaka bir KBB (Kulak Burun Boğaz) Hastalıkları uzmanına başvurulmalıdır‼️
✅ 2 haftayı geçen ve tedavi (antibiyotik+iltihap kurutucu) ile küçülmeyen her boyunda kitle, aksi ispatlanana kadar KANSER olarak düşünülmelidir.
