Meme Ameliyatlarında Önemli Pivot Noktalar

Meme ameliyatlarının tümünde memenin önemli noktaları bulunuyor. Memede bir asimetri ortaya çıkmaması, memelerin genel görüntüsünde bir farklılık ortaya çıkmaması için bu noktalara çok dikkat etmek gerekiyor. Sizlere genel olarak ölçümlerden bahsettim. Ancak, bazı noktalardan da bahsetmek istiyorum. Memede en önemli noktalardan bir tanesi meme altı oluğunun bulunduğu bölge. Meme altı oluğunun bulunduğu bölge her kadında çok farklılık gösteriyor. Meme altı oluğu eğer yukarıda konumlandıysa o zaman memeler daha çok tüberöz meme yapısında oluyor. Yani, meme altı oluğunun yukarıya doğru çıkması ve memenin alt kısmının gelişmemesiyle tüberöz meme adını verdiğimiz keçi memesi şeklinde yukarıdan aşağıya fırlak bir meme ortaya çıkmakta.

Meme ameliyatlarında dikkat ettiğimiz ölçümler ve önemli noktalardan bahsetmeye devam etmek istiyorum. Jugulum ile meme ucu, memu ucu ile meme altı oluğunun mesafelerinden bahsetmiştim. Bugün de sizlere meme ucunun orta hattan olan uzaklığından bahsetmek istiyorum. Bu, kadınları ilgilendiren en önemli ölçümlerden bir tanesi. Meme ameliyatlarında dikkat ettiğimiz önemli noktalar ve ölçümlerden bahsediyordum sizlere. Jugulum ile meme ucu mesafesini anlatmıştım. 19-21 cm arası olması gerektiğini iletmiştim. Ama kişiden kişiye göre bu mesafenin değişebileceğini söylemiştim. Gövde uzunluğu fazla olan bireylerde bu mesafe uzayabilir, gövde uzunluğu kısa olan bireylerde bu mesafe daha kısa olabilir. Sadece bu uzunluğa göre meme ucu mesafesine karar vermiyoruz. Farklı kriterler de var. Bunlardan da sizlere bahsedeceğim. İkinci önemli mesafe meme ucu ile meme altı oluğu arasındaki mesafe. Meme ameliyatlarından sonra memenin üzerinde kalan izlerin ameliyat öncesinde tasarlanması çok büyük önem arz ediyor. Özellikle karın bölgesi ve memenin geçiş hattında bulunan izlerin iyileşmesi kişinin vücut özellikleri, genetik özellikleri ve çevre faktörlerinden etkilenmekte. Burada meme altında konumlandırılan izin meme altı oluğunun biraz üstünde konumlanmasına gayret ediyoruz. Her zaman bu geçerli olmayabiliyor. Özellikle karın bölgesi gergin olan hastalarda bu izi biraz yukarıya taşımak çok zor olabiliyor ve taşıyamayabiliyoruz. Aslında, normal kilosunda olan bireyleri ameliyat etmeyi tercih ediyoruz. Kilosu yüksek olan bireylerin kilo verip ameliyat olmalarını öncelikli olarak tercih etmekteyiz. Onkoplastik Cerrahi İle Kombine Meme Küçültme⁣

Burada görmüş olduğunuz gibi kitlenin bulunduğu yere göre özellikle kitlenin çıkarıldığı bölgenin doldurulacağı şekilde bu ameliyatı planlamaktayız. Buna terapötik mamoplasti yani tedavi edici meme küçültme cerrahisi adını veriyoruz. As you can see here, we plan this surgery according to the location of the mass, especially in a way that the area where the mass is removed will be replaced. We name this case as a therapeutic mammoplasty. In order to close the defect caused from excision of the mass at the lateral side of the ni**le (i.e at the outer quadrant of the breast), we create a glandular tissue attached to the ni**le and shift it to that area. It is ensured that the depresssion and the cavity may cause in that area are prevented with this method. By this way, tumour surgery and breast reduction can be combined very successfully.⁣
