This patient presented with a nasal tip deformity and a collapsed nasal structure following previous surgery.
During revision rhinoplasty, the tip was carefully reshaped and the collapsed nose was structurally elevated to restore proper support.

Even on the operating table, a smoother, more balanced and natural nasal contour is clearly visible.

Nasal Septum Perforation Closure Surgery 2nd Week Postoperative Result

- The perforation was caused bu co***ne use.

🌎 Worldwide, the success rate of septal perforation closure surgery isnapprox. 60-70%.

🔬 With the surgical technique developed by Prof. Dr. Gediz Murat Serin and published reached around 90%.

- Howewer no surgical procedure can be guaranteed with 100% success.
- Even when the surgery is performed perfectly, about 1 out of 10 patients may experience reopening of the perforation.

❗️This doesn't necessarily indicate a surgical error.
Just as septal perforation may rarely occur after rhinoplasty without surgeon fault, individual healing capacity, tissue quality, and blood supply are decisive factors.

🩺 Septal Perforation: When the Cause Is Not Obvious

Not all nasal septal perforations have a clear or single cause.

While co***ne use and previous nasal surgery are well-known risk factors, many patients do not have these histories.

In daily practice, common non-rheumatologic contributors include:
• Childhood nasal trauma
• Chronic nose picking
• Long-term use of topical nasal sprays
• Recurrent infections
• Subtle mucosal ischemia over time

In some patients, these factors may act silently and cumulatively, leading to perforation years later.

📌 Even when major causes are excluded,
“idiopathic” perforations may still have an underlying trigger — just not an obvious one.

Understanding this helps avoid unnecessary blame, delays in diagnosis, and missed treatment opportunities. ***nenose

A complication. A reconstruction. A solution.

A patient previously underwent rhinoplasty and developed a septal perforation secondary to chronic infection caused by permanent Prolene sutures.
An initial repair attempt failed — and the perforation enlarged further.

In this revision case, the key was not only closure, but rebuilding a healthy, vascularized environment.

✔ Complete removal of all infected permanent sutures and fibrotic tissue
✔ Reconstruction using an anterior ethmoid artery–based flap
✔ Interposition graft with fascia lata
✔ Multilayer, vascularized closure

This approach is especially valuable in revision, infected, and complex septal perforations, where tissue quality is compromised and standard techniques fail.

SEPTAL PERFORATION REPAIR

This case involved 2 perforations, both anterior, in a nose that had already undergone two previous surgeries. The septal tissue was severely compromised, with almost no remaining tip support, and permanent sutures were found at the perforation margins. These non-absorbable sutures may have contributed to chronic inflammation and infection, ultimately worsening the tissue loss. In revision septal surgery, leaving permanent foreign material can significantly reduce healing potential. By avoiding permanent sutures and using carefully planned rotational and advancement flaps, both perforations were successfully closed while respecting nasal physiology and vascularity.

✨ Large Septal Perforation Repair – Post-Surgical (2.7 cm) ✨
This patient had a 2.7 cm septal perforation caused by a previous nasal surgery, located deep in the posterior septum.
Repair was performed using a 3-layer closure technique, restoring both structural support and full mucosal continuity.

In the post-operative video, the nasal cavity appears clean, closed, and healing smoothly, demonstrating excellent early results.
