Septal perforations are most commonly caused by previous nasal surgeries or co***ne use; other causes are much rarer. Repairing a perforation is a technically demanding operation and not something performed routinely by many surgeons.

In my practice, however, septal perforation repair is part of my weekly surgical routine, and I use advanced, multi-layer techniques tailored to each patient.

Revision Septal Perforation Repair
A previous surgery had failed to close the patient’s septal perforation, leaving persistent crusting, bleeding, and breathing difficulties. In this revision procedure, I performed a multi-layer reconstruction using a mucosal rotation flap, an anterior ethmoidal artery–based flap, and a fascia lata interposition graft — a technique I have personally refined over the years. This combination allows us to rebuild the septum with strong vascular support and achieve a stable, long-term closure.

🇬🇧 Septal Perforation Closure After Previous Septal Surgery
Secondary septal perforations—often caused by previous septal surgeries—are among the most challenging reconstructive cases in nasal surgery.
In this patient, the nasal septum was severely thinned and scarred due to prior intervention. Using a double mucosal rotation flap combined with fascia lata interposition, we successfully achieved full closure and restored both nasal function and comfort.

Each case of septal perforation is unique, especially when there’s a history of surgery. Careful planning, meticulous flap design, and multi-layer closure are key to long-term success.

Advanced Traumatic Nose – 3 Weeks After Combined Surgery
This patient had a severely deformed nasal structure following trauma.
We performed a combined functional rhinoplasty and endoscopic sinus surgery, restoring both nasal symmetry and sinus ventilation.
Using Piezo ultrasonic technology, bone reshaping was done with exceptional precision and minimal trauma. Travma sonrası ciddi kemik deformiteleri bulunan bu hastada, fonksiyonel rinoplasti ve endoskopik sinüs cerrahisini birlikte uyguladık.
Piezo ultrasonik teknoloji ile kemik şekillendirmesi son derece hassas ve travmasız şekilde gerçekleştirildi.
Sadece 3. haftada, doğal, dengeli ve fonksiyonel olarak mükemmel bir sonuç elde ettik.

Cocaine-induced septal perforation represents one of the most challenging areas in nasal reconstruction. In this patient, a previous surgery had failed to achieve full closure of the defect.
Using my own technical modification, I performed a three-layer repair consisting of a mucosal rotation flap, an anterior ethmoidal artery–based flap, and an interposition of fascia lata between them — restoring both structural integrity and mucosal continuity.

Diş implantları dişsiz bölgelerde protez yapımı için kullanılan güvenli yöntemlerdir. Ancak üst çene bölgesinde, implantın maksiller sinüse çok yakın olması bazen komplikasyonlara yol açabilir.
İmplant sinüs boşluğuna ilerleyebilir veya sinüs ile ağız arasında bir açıklık (oroantral fistül) gelişebilir.

Bu hastamızda her iki durum da mevcuttu. Ameliyatta endoskopik sinüs cerrahisi ile sinüs içindeki enfekte dokuları temizledik, implantı ve fistül bölgesini çıkardık.
Bu tür olgular, multidisipliner yaklaşım ve cerrahi deneyim gerektiren karmaşık durumlardır.

Dental implants are reliable solutions for restoring missing teeth, but in the upper jaw they lie very close to the maxillary sinus, which sometimes leads to complications.
The implant may migrate into the sinus or form an abnormal opening (oroantral fistula) between the mouth and the sinus.

In this patient, both conditions were present. We performed endoscopic sinus surgery, cleared the infected tissue, and removed both the implant and the fistula tract.
Such cases require surgical expertise and multidisciplinary collaboration for successful outcomes.
