What is it in Vitro Fertilization (IVF) Treatment
In vitro fertilization is an assisted reproductive method that involves the fertilization of egg cells (oocyte) and semen (s***m) taken from the woman in a laboratory environment and the placement of the fertilized embryo into the uterus of the woman. Pregnancy obtained from a certain number of embryos selected and placed in the uterus progresses in the same course as the pregnancy achieved by normal means. In this sense, the only difference between natural pregnancy and IVF treatment is that fertilization is carried out in laboratories with suitable conditions.

The first successful IVF treatment case, founded by Robert G Edwards in 1971, was seen in England in 1978. The first successful pregnancy in Turkey was concluded with the in vitro fertilization method at Ege University in 1989. Thanks to the development of medical techniques and technologies, different in vitro fertilization techniques are applied, and successful results are obtained.

Selecting the correct s***m(s) for microinjection during assisted reproductive technologies seems one of the key points for success.

Despite all the improvements in laboratory procedures and clinical in vitro fertilization treatments, pregnancy rates with IVF are still relatively low. Successful implantation of the embryo requires a normal shaped uterus with a proper and receptive endometrium ( inner layer of the womb ) and a genetically healthy embryo.

The relatively high rate of embryo aneuploidy ( abnormality in number of chromosomes ) seems to limit the implantation and pregnancy rates. As a woman ages, the risk for embryo aneuploidy increases. We have learned from the studies on embryos that most of the genetic abnormalities are of maternal origin, but the s***m also contributes to the genetic content of the embryo, and therefore some problems are of paternal origin. We also know that the lower the s***m count, the higher the risk for a genetic etiology of the abnormal s***m production.

In IVF, the number of the eggs from women are mostly limited. The number of the s***ms from men is much higher respectively. Therefore, it is crucial to select the appropriate s***ms for ICSI. There have been different s***m selection tools recommended for that aim. The biochemical and physical characteristics of mature, fully developed s***ms differ from those of immature, genetically unhealthy s***m. Mature s***m display hyaluronan binding sites, and this ability has been shown to be a potentially useful tool during the selection of s***m for ICSI ( PICSI ).
Microfluidic tunnel system is another selection method used for sorting s***ms which have better morphology, genetic and physiologically competent.
Men with very low motility s***ms are also offered to undergo testicular s***m aspiration with fine needle since collecting the s***ms directly from testickles have been found more succesful than using the ej*****te.
