Closed Revision Rhinoplasty
One of the questions I get most often is whether I perform revision (secondary) rhinoplasty. About 10% of my cases are revision surgeries, and I most commonly perform them using an open approach. Still, just like in primary cases, I prefer to start revision procedures with a closed approach, and convert to an open approach if needed. This helps me preserve the blood supply to the skin as much as possible.
Performing revision surgery with a closed approach unquestionably requires significant effort and experience. In patients where the risk to skin perfusion is considered high, it may actually be appropriate to push for this option, as it can help keep the risk lower by protecting the skin’s blood supply. However, when necessary—and in cases where the overall risk is relatively low—the open approach is often the method I rely on more frequently for revision rhinoplasty.

How can a surgical procedure change your appearance?
I’ve always believed that surgical enhancements should make you look more energetic, refreshed, and like the best version of yourself. But surgery should never turn you into someone else. Having a very “young-looking” face may simply make you look younger—but is beauty really just about looking young?

One of the most challenging deformities to correct in rhinoplasty is, without a doubt, the crooked nose. The human body is not naturally designed to be perfectly symmetrical—so neither is the face. The anatomical structures that support the nose on both sides, as well as above and below, are not positioned in exactly the same plane. For this reason, you could say that there is essentially no nose in the world that is perfectly straight.
In crooked nose rhinoplasty, we take all of these factors into account and aim to achieve the most symmetrical result possible. I always discuss these points openly during the preoperative consultation so that my patients’ expectations are aligned with what is realistically achievable.
This patient had a markedly crooked nose, thin skin, and weak alar sidewalls. As I do in almost every primary case, I used a closed rhinoplasty approach. I come across speculation about this topic quite often. Of course, in revision surgeries or extremely challenging noses, an open approach can be used—and I use it when needed. However, in more than 95% of primary cases, I can achieve what I want with closed rhinoplasty. Therefore, the claim that the closed approach does not provide sufficient visualization is simply not accurate.
The piezo instruments we use for bony work are a routine part of my practice, but “piezo” is not a technique—it’s a tool. For correcting crooked noses, I use my own Septal Redirection technique together with a structural rhinoplasty method.

Almost 4 years have passed 🧿✨

A surgeon’s greatest wish and source of joy, I believe, is to develop a technique that will be remembered by name. With more than ten years of experience in rhinoplasty, like many of my colleagues, I have refined my own methods—and I feel I have now reached the maturity to share them. I am proud to publish the “Footplate Graft” technique, which I have used for many years in short noses, especially those with an acute nasolabial angle. My friend .sinanhaddad ’s efforts and support were invaluable in turning this technique into an engaging manuscript and getting it published in the prestigious The Official Journal of the American Academy of Facial Plastic and Reconstructive Surgery (AAFPRS) ,and I would like to sincerely thank him as well. I hope to continue contributing my original techniques to the literature as much as I can in the coming period.

I’ve noticed that many Black patients may want rhinoplasty, but they don’t want to completely lose their ethnic features. When I ask what they want, a lot of them say they’d like a more defined nasal tip and correction of tip drooping—without looking like a “white nose.” This patient was exactly like that, and I believe we were able to give her a better nose while preserving her ethnic character.
In these types of cases, I’d like to emphasize that a approach can be possible. I often prefer it because the risk of visible scarring is low and recovery can be faster. For this patient, we harvested and used a rib cartilage graft, and a 2.5 cm incision was sufficient.
Of course, these decisions are shaped partly by preference and partly by necessity. In patients with a very weak nasal dorsum, it may be essential to augment the bridge as well. And a closed approach isn’t suitable for every patient—sometimes that decision depends on what we encounter and how the surgery evolves in real time.
