【2020除栓論壇回顧系列5】

✅主題：「undated treatment of pulmonary embolism guideline review」

某個平安夜的夜晚急診救護車送進一位肺栓塞的孕婦到院前已無生命跡象，急診醫護人員持續做CPR搶救，心臟外科團隊緊急放上葉克膜，並且做了血栓溶解劑治療，終於讓這位孕婦順利保住性命並在48小時後移除葉克膜。這節課程張兼華醫師將帶大家了解pulmonary embolism(肺栓塞)診斷與治療，從2019年ESC最新治療指引中新增與修正不少過去的治療指引，包含高風險肺栓塞患者的入院流程、NOACs(新型抗凝血)藥物的使用等等……，在治療計畫中相當重要的環節就是下診斷，指引中推薦的D-dimer、CTPA、VQ scintigrapny、Leg cus，其中要注意的是若是臨床上已經高度懷疑有肺栓塞的患者就不適合使用D-dimer……

#小編的話：
急性肺栓塞若沒妥善處置致死率高達30%，肺栓塞臨床上的症狀常常伴隨呼吸困難、急促或咳嗽、咳血、胸痛，也因此常常被誤以為是心臟或其他問題而忽略，這也是肺栓塞之所以可怕又致命的原因之一，幸好近年來在醫界重視與努力下提升臨床教育與診斷工具逐漸改善這個問題，目前醫療技術一但確定診斷並以導管除栓或藥物治療都能達到不錯效果。預防肺栓塞要注意:1.避免久站久坐，適時起來走動增加血液循環2.若血管曾受傷或是有下肢靜脈血栓，應定期追蹤及遵照醫囑服用抗凝藥物，預防勝於治療才是避免肺栓塞找上門的方法。

根據統計在中央狹窄患者6個月的通暢率為60%，但到了12個月通暢率僅剩30%，但支架並非中央靜脈狹窄最好選擇，何時該置放支架、該選何種尺寸支架、支架放置有無確實放置病灶處，都是支架能否維持暢通率的關鍵，陳偉華醫師分享如何透過IVUS(血管內超音波)的輔助經驗，使得術後的通暢率有更亮眼的表現……

According to statistics, the patency rate of patients with central vein stenosis at 6 months is 60%, but by 12 months, the patency rate is only 30%. Stent isn’t the best choice for central vein stenosis. When should we place the stent? What size stent should we choose? Whether the stent is actually placed on the lesion? The above is the key to whether the stent can maintain the patency. Dr. Jimmy Wei Hwa Tan shared how to use IVUS (Intravascular Ultrasound) assisting experience to make the patency rate more brilliant after the operation.

血液透析導管往往是急性腎衰竭患者在急需洗腎時最優先的選擇，但血液透析導管對大部分的透析患者來說並非最佳選擇，根據臨床指引導管的置放建議越早移除改為手上管路透析為佳，因此這堂課程將針對血液透析導管併發症有更全面且深入的解說。

Hemodialysis catheters are often the most preferred option for patients with acute renal failure when they urgently need dialysis. However, hemodialysis catheters are not the best choice for the majority of dialysis patients. According to the guidelines, the sooner the hemodialysis catheter is recommended to be removed, it is better to switch to AV shunt. Thus, this course will provide a more comprehensive and thorough explanation of the complications of hemodialysis catheters.

Dr. Hong Ye這堂課程主要分享血液透析導管相關併發症，常見的導管無功能、導管脫落、感染到罕見的空氣栓塞等等……，也從解剖位置及組織學角度建議置放時導管尖端位置避免血栓形成。對於移除導管時最擔心的就是遇到導管卡住情形，因此Dr. Hong Ye 也提到各種血管介入和外科手術進行移管。

Dr. Hong Ye mainly shares the complications related to hemodialysis catheters, common hemodialysis catheters have no function, hemodialysis catheters fall off, infections, and rare air embolisms... She also suggested the placement of the hemodialysis catheter tip from an anatomical location and histological point to avoid thrombosis. The most worried about is the hemodialysis catheter stuck when removing the it, so Dr. Hong Ye mentioned various vascular interventions and surgical procedures for hemodialysis catheter moving.

內膜增生是狹窄發展的主要病理基礎，對於內膜增生問題醫界也不斷找尋各種應對方法，例如:切割球囊血管成形術、裸金屬支架、覆膜支架、藥物塗層球囊血管成形術等等……，對於切割球囊與塗藥球囊Dr. Tay Kiang Hiong做了更進一步的研究，將只使用切割球囊或只使用塗藥球囊治療的患者，對比同時使用切割球囊與塗藥球囊治療患者的術後通暢率，呈現客觀的結果與結論。

Hyperplasia is the main pathological basis for stenosis, and the medical community is constantly looking for various methods for the problem of it, such as cutting balloon angioplasty, bare-metal stent, stent-graft, drug coated balloon angioplasty, and so on. Dr. Tay Kiang Hiong did further research on cutting balloon and drug coated balloon. He will compare postoperative patency treated with cutting balloons or drug-coated balloons only to the simultaneous use of cutting balloon and drug coated balloon to treat patients, and provide objective results and conclusions.

透析護理品質增加讓透析患者的平均餘命延長，但也因血管內介入處置次數變多導致上半身的靜脈容易產生狹窄或阻塞，以至於透析患者身上有不只一個透析通路。辛俊賢醫師分享一位深受靜脈狹窄困擾的透析患者，來接受英雄導管的置放過程。

Increased quality of dialysis care extends the average life of dialysis patients. However, due to the increased treatment number of vascular interventions, the veins of the upper body are prone to stenosis or occlusion. So, there is more than one dialysis access in dialysis patients. Dr. Chun-Hsien Hsin shared that a dialysis patient who accepted the Hero Graft placement process deeply troubled by venous stenosis.
