陽明交大醫院醫療科技展發表智慧精準醫療成果

國立陽明交通大學附設醫院12月4日至7日將在「台灣醫療科技展」展出多項創新成果，發表微創手術與精準醫療領域的最新突破，包括全台少有的智慧複合式手術室、骨科微創電腦導航，以及猶如「牙科GPS」的3D植牙導航系統，展現邁向智慧醫療里程碑，讓病人手術時更安全、術後恢復更快速。

面對植牙潛在風險，陽明交大醫院引進「X-Guide 3D 植牙導航系統」，張妤欣醫師說，這套系統猶如「牙科GPS」，手術中利用光學追蹤，即時導引器械和人工植牙方向，給醫生更準確的資訊，提供比傳統精準11倍的植牙手術，不僅安全快速完成微創植牙，術後傷口小，降低腫脹疼痛，大幅縮短診療時間。

院長黃志賢表示，醫院引進多項最新儀器，目的是為了守護鄉親健康，帶進先進智慧醫療，讓民眾無需遠赴外縣市，就能享有最高品質、低風險、恢復快的治療服務；陽明交大醫院的每一步成長都是對民眾健康的承諾，也是責無旁貸的使命。

KODAMA Guideline：Evidence-based Collagen Application to Tissue Management
児玉準則：膠原蛋白使用於組織管理的實證醫學

Prof. Kodama根據30多年的材料與臨床研究，將拔牙創口的組織缺損，做以下分類：

軟組織
Type I：軟組織完整
Type II：軟組織缺損，MGJ以上
Type III：軟組織缺損，MGJ以下

硬組織
Type A：頰側骨板完整
Type B：頰側骨板缺損，小於1/2
Type C：頰側骨板缺損，大於1/2

KODAMA Guideline根據不同軟硬組織缺損，建立因應的組織管理策略：

Suture：單純縫合
Teruplug：單顆膠原
Pile-up technique：Teruplug + Terudermis 膠原+人工真皮
Outside technique：Double Teruplug雙側膠原

Prof. Kodama遵從牙周生理和再生機制，創造「組織再生」環境，利用膠原蛋白作為「再生平台」，搭配牙周手術，重建拔牙創口的軟硬組織。

案例分享：
根據Prof. Kodama分類法，這是屬於Type II+C的案例（軟組織缺損不超過MGJ，頰側骨板缺損超過50%），採取Pile-up technique處理，三週後軟組織癒合重建，六個月後頰側骨再生完整，後續進行植牙治療，一年半的追蹤，植體和周圍組織都維持穩定。

KODAMA Guideline不僅是一個治療準則，也是教育訓練方針，藉由基礎課程和Hands-on實作，讓更多醫師學習，受惠於需要拔牙合併植牙的病患。

This case shows Ribbond closely bonded against the surfaces and contours of the cavity using the Dr. Chyz technique. Close placement of the Ribbond against the tooth minimizes the composite thickness between the fibers and the tooth, which minimizes the volume of composite resin that can shrink and maximizes the fiber laminate strengthening effect.

Many dentists find the Dr. Chyz technique easier for closely placing the Ribbond against the tooth than the composite-on-the-tooth-first Ribbond technique. The Dr. Chyz technique flattens a soft-past viscosity anterior restorative composite on a mixing pad, the Ribbond is wetted with an unfilled bonding adhesive and gently placed into the flattened composite. The Ribbond-composite piece is carefully removed from the mixing pad by incrementally sliding a thin bladed instrument under the composite and then placed into the tooth with the composite side against the tooth.

Composite resins shrink especially in cases of high C-factor and one of the main purposes of adding Ribbond is to prevent gap formation between the restoration and the tooth. Ribbond will have a positive effect where it is placed and the more surface area covered with Ribbond, the better the results will be. This photo is a great example of very good surface coverage of Ribbond bonded closely on the pulpal floor and on the cavity walls.

Sometimes it takes a piece of Ribbond on the pulpal floor and an additional piece or two of Ribbond on the cavity walls to maximize coverage.

Keep the Ribbond approximately 1.5mm below the cavo-surface margin.
